Re: Operating Budget
The State of Hawaii Department of Health, Diabetes Prevention and Control Program
has awarded the Hawaii County Office of Aging a grant in the amount of$7,144 to be
used to continue implementation of the Chronic Disease Self Management Program
(CDSMP) by facilitating and implementing training of Lay Leaders for the Diabetes Self
Management curriculum (DSMP).
Enclosed is a bill for an ordinance amending the Operating Budget by increasing the
appropriation of the Area Plan on Aging Account by $7,144.
The Office of Aging is requesting that this Resolution be waived from the Finance
Committee and be placed on the April 4, 2012 Council Agenda. If there are any
questions, please do not hesitate to call Brenda Isa of the Office of Aging at 961-8600.

B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED):
Grant funds (Diabetes prevention and Control Program, DOH) awarded to Hawaii County Office of Aging to
continue implementation of the Chronic Disease Self Management Program (CDSMP) by facilitating and
implementing training of Lay Leaders for the Diabetes Self Management curriculum (DSMP). Costs include
airfare and per diem and ar rental for two DSMP Master Trainers fro another neighbor island; training
materials and supplies; and travel costs and per diem for lay leaders that could include those from Kona,
Ka`u, and Waimea.

RE: Bill No. 220 : A Bill For An Ordinance Amending Ordinance No. 11-
59, As Amended, The Operating Budget for the County of Hawai`i for the Fiscal
Year Ending June 30, 2012; Distribution of Funds from State Grants — Area Plan
on Aging Account to Area Plan on Aging Account ($7,144 — funds will be
used to continue implementation of the Chronic Disease Self Management
Program (CDSMP) by facilitating and implementing training of Lay Leaders for
the Diabetes Self Management curriculum (DSMP)
Pursuant to Section 2(g) of Rule 4 of the Rules of Procedure of the Council of the County of
Hawai`i, this written request is submitted with my approval that the above-referenced matter be
waived from the Committee on Finance to the full Council for immediate action. In reviewing
this matter, timely approval is crucial. It is therefore advantageous that approval is granted and
the matter be placed onto the next Council agenda for review. However, in the event this request
is denied, for whatever reason, I understand the matter shall be referred to the Committee on
Finance for placement on its future agenda.
