RESOLUTION NO. 238 12 RESOLUTION AUTHORIZING THE OFFICE OF THE MAYOR TO ENTER INTO AN AGREEMENT WITH THE STATE OF HAWAII DEPARTMENT OF HEALTH, ALCOHOL AND DRUG ABUSE DIVISION, PURSUANT TO HRS 46-7, FOR A GRANT FOR THE COUNTY OF HAWAII LIQUOR CONTROL DRUG, ALCOHOL AND TOBACCO AWARENESS PROGRAM WHEREAS, the State of Hawaii Department of Health has authorized federally derived funds for the Department of Liquor Control for use by the Department's Drug, Alcohol, and Tobacco (D.A.T.)Awareness program; and WHEREAS, student-athletes from Kea'au High School have expressed a desire to create a video called "If I Neva Do D.A.T. "; and WHEREAS, this video will be composed of a series of vignettes highlighting the consequences of using alcohol, tobacco and drugs; and WHEREAS, the funds from this grant will be used to contract with "daBraddas", who will provide direction to these student-athletes in creating and producing the video; and WHEREAS, Hawaii Revised Statutes, Section 46-7, requires that County departments obtain the consent of the Council to enter into agreements with the Federal or State governments respecting action to be taken pursuant to any of the powers granted by law to furnish, expend, and receive any funds or other assistance in connection with projects being or to be undertaken pursuant to those powers; now, therefore, BE IT RESOLVED BY THE COUNCIL OF THE COUNTY OF HAWAII, in accordance with section 46-7, Hawaii Revised Statutes, that the Mayor of the County of Hawaii is authorized to execute, on behalf of the County, an agreement with the State of Hawaii Department of Health, to enable the County to implement the production of the "If I Neva Do D.A.T." video as indicated in the proposed operating agreement, attached hereto and incorporated herein by reference as Exhibit "A", and related documents. BE IT FINALLY RESOLVED that the County Clerk of the County of Hawaii shall transmit copies of this resolution to the Office of the Mayor, the Department of Liquor Control, and the Finance Department. (Drug,Alcohol, and Tobacco)Awareness Program Project Sponsor: Hawaii Partnership to Prevent Underage Drinking (HPPUD) Statewide Coalition Project Manager: Jan Yokoyama Date: November 1, 2011 Purpose of the Teaming Agreement(TOA) This TOA serves as the guidelines and ground rules to help the project team work most productively together over the course of the project. The TOA is a living document and may be updated as needed. Groups must sign this TOA before receiving funds. This is a one-time, non-reimbursable award to support the coalitions, and should not be considered a long-term funding source. If there are any changes to your proposed project you must send in a revived proposal approved by the HPPUD Council Board Members. Project Overview County of Hawaii Liquor Control, D.A.T. (Drug,Alcohol,and Tobacco)Awareness Program,has been approved and awarded on the(7,11,11) in the amount of FIVE THOUSAND DOLLARS AND NO CENTS ($5,000.00). In 2011, the Hawai'i County D.A.T. (Drug, Alcohol and Tobacco)Awareness Program, under the direction of Jan Yokoyama and Iris McGuire, have trained 175 student-athletes from 14 private and public high schools throughout Hawai'i island . These student-athletes are committed to conduct Reach Out Now Teach-ins, a SAMHSA curriculum,for 5th and 6th graders in their feeder schools. The initial Teach-ins conducted in April and May reached 2644 students. Another round of Teach-ins will start in January 2012. This program is sponsored by the Hawai'i County Department of Liquor Control in collaboration with the Big Island Interscholastic Federation (BIIF). To assist with sustainability of this program, the student-athletes from Kea'au High School have expressed a desire to create a video called"If I Neva Do D.A.T." This video will be composed of a series of vignettes highlighting the consequences of using alcohol, tobacco and drugs. The student-athletes are creating and acting in the vignettes. Once the video is completed, the BIIF will be sharing it with all their participating schools, along with their respective feeder schools. The grant funds will be used to contract with daBraddahs who will provide direction to the student-athletes in creating and producing the video. Budget The awardees are to keep track and submit original receipts(not copies) of all funds used for this project to the HPPUD Statewide Coalition office for audit purposes. Please submit your first set of receipts February 1,2012 and remainder of your receipts March 31,2012. Evaluation/Progress Reports You also agreed to provide two (2) reports which the second progress report will be your project evaluation report and final report. The first progress report is due February 1, 2012. Please create a document answering the following questions: 1. Please explain how you have engaged youth as leaders in the implementation of this project? 2. What steps in your proposal have you completed and what steps do you still need to complete? 3. Of the steps that you still need to complete, do you expect to finish them within your proposed timeline? If not,when do you expect to complete them? 4. How many meetings, events, etc., have occurred and how many people participated? *Please provide a full breakdown of how the budget was spent thus far. The final progress report and project evaluation is due March 31,2012. Please create a document answering the following questions: 1. How do you know that you were successful in completing your project? 2. How do you know that your project had an impact in preventing underage drinking, or in changing the awareness, attitudes, or behaviors related to underage drinking? 3. Are there any activities/steps that you didn't complete? If yes, please explain why. 4.
