Re: Resolution Accepting Donation
Enclosed is a resolution accepting the donation of two Automated External Defibrillators (AEDs),
valued at$2,708 from the Hawaii Heart Foundation, P.O. Box 542, Kailua, Hawai`i 96734. This
equipment will be utilized by the Hawaii Fire Department's Volunteer Division to support their
mission to increase the survival rate from community cardiac arrest within the Island of Hawaii.

A. REQUEST:
To accept donation of two Automated External Defibrillators (AEDs), valued at $2,708.00 but, does not
include carrying case. Although these AEDs are not new, they have never been used and were well
maintained. The Philips Onsite HS1 AEDs devices have passed all self tests and will need pads and
batteries in order to be placed in service. Please send a letter of appreciation to:

B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED):
These two AEDs are a donation by the Hawaii Heart Foundation so that they may be placed into the
community for use in an emergency. Donation is to benefit a portion of the Hawaii Fire Department's
mission of increasing the survival rate from community cardiac arrest within the Island of Hawaii. These
units will be utilized by our Volunteer Division.

RE: Resolution No.259-14 : A Resolution Authorizing the Acceptance of a Donation of Two
(2) Automated External Defibrillators for Use by the Hawai`i Fire Department.
Pursuant to Section 2(g) of Rule 4 of the Rules of Procedure of the Council of the County of
Hawai`i, this written request is submitted with my approval that the above-referenced matter be
waived from the Committee on Finance to the full Council for immediate action. In reviewing
this matter, timely approval is crucial. It is therefore advantageous that approval is granted and
the matter be placed onto the next Council agenda for review. However, in the event this request
is denied, for whatever reason, I understand the matter shall be referred to the Committee on
Finance for placement on its future agenda.
