Date Form #: B-52 7/18/91 DEPARTMENT OF FINANCE REQUEST FOR COUNCIL ACTION DEPARTMENT: FIRE DATE: 12/18/13 STAFF CONTACT: Jesse Ebersole PHONE: 961-8590 A. REQUEST: Please prepare a resolution to accept a monetary donation of$750.00 from Hilo Walmart and please send a letter of appreciation to: Walmart 702 SW 8th Street Bentonville, AR 72716 B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): Each year more than 1/3 of our population, age 65 and over, suffers a fall. These falls account for one out of every 10 emergency department visits in our country and over $20 billion in healthcare costs each year. Specifically within the Hawaii County, this equates to over 1,400 emergency department visits, over 400 hospital admissions, and seven deaths each year. In a partnership with Hilo Medical Center, State Emergency Medical Services and Injury Prevention, and Hawaii County Office of Aging (ADRC), the Hawaii Fire Department is preparing to rollout a Senior Fall Prevention and Home Safety Program in which EMS paramedics will perform a standardized fall risk assessment, complete a fall prevention home safety checklist, provide fall safety education, and when necessary make referrals within the community healthcare system to assure patient safety, prevent falls, and ultimately improve outcomes and quality of life for the Kupuna on Hawaii Island. This funding from Walmart will aid in the purchase of a laptop computer to assist in management and data collection for the EMS Bureau's Fall Prevention Program.
