J�tV Os M,�Y <br /> COUNTY OF HAWAII ••` =• STATE OF HAWAII <br /> art Cf•M►•� <br /> RESOLUTION NO. 435 14 <br /> A RESOLUTION APPROVING PARTICIPATION BY THE HAWAII STATE <br /> ASSOCIATION OF COUNTIES IN THE NATIONAL ASSOCIATION OF COUNTIES' <br /> HEALTH DISCOUNT PROGRAM. <br /> WHEREAS, the County of Hawaii is a member of the National Association of Counties <br /> ("NACo") and the Hawaii State Association of Counties ("HSAC"); and <br /> WHEREAS, NACo is providing a health discount card at no additional cost for member <br /> counties and state associations of counties to offer to residents; and <br /> WHEREAS, the cards will be marketed by CVS Caremark for a subscription fee of <br /> $6.95 monthly per individual ($69 annually) or $8.95 monthly per family ($79 annually); and <br /> WHEREAS, card holders will benefit from discounts on vision care, laser eye surgery, <br /> prescription drugs, diabetic supplies, lab testing services, diagnostic imaging services and <br /> hearing aids from a broad network of medical suppliers; and <br /> WHEREAS, card holders will have no annual limits on use, no forms to fill out, no <br /> waiting periods, no age or income requirements and no medical condition restrictions; and <br /> WHEREAS, participation by HSAC requires approval of the marketing agreement as <br /> provided in the attached Exhibit "A" by its member counties; now, therefore, <br /> BE IT RESOLVED BY THE COUNCIL OF THE COUNTY OF HAWAII that <br /> HSAC participation in the NACo Health Discount Program as provided in the attached Exhibit <br /> "A" is approved. <br />
