Subdivision No. 90-017
Location: Kukuau, South Hilo, Hawaii

A dedication field inspection has been completed and we recommend deferral of
dedication until the following are rectified.
Wastewater System:
1 Repair/Replace 8-inch VCP sewer main. There are twenty-one (21) substantial
cracks to date as noted by CCTV inspection.
2. Install missing sewer lateral stub-out markers (1-1/2" PVC w/ 12"x12" concrete
collar) at Lots 3,7,46, and park site.
3. Install Type SA rung (one per manhole) on all sewer manholes.
4. Rurg spacing in all cower marhotes do rct comply with Standard Datail 5-11.
5. Install missing Type SE rungs in SMH B-6.
6. Test sewer system for leakage in compliance with Section 18.3.D. of the
Standard Specifications.
Traffic Signs and Marking:
All signs shall he high intensity sheeting.
Kahakina Street (Street "13") at Ohukea Street:
7. Repaint double yellow centerlines.
8. Replace two (2) End of Road Markers. Refer to MUTCD for size.
9. Replace missing Dead End sign (W14-1 .

The owner(s) shall request of the Department of Public Works a re-inspection once the
above items have been rectified to determine whether repair, maintenance, or Cleaning
is necessary prior to dedication.
