Subject: The Violence Intervention Project of Bay Clinic, inc.
Human Services Non-Profit Grant.
Please find enclosed a revised budget for the FY 99-00 grant with
the following clarifications:
* Table 1; Total Agency Budget FY 98-99 and Total Agency
Budget FY 99-00: No names or salaries are listed in these
columns because there is a clear distinction between Bay
Clinic, Inc. and the Violence Intervention Project, with no
staff members in common. Bay Clinic,Inc. is the umbrella
~ agency for VIP.
~ * Table 1; The salary of Program Manager/Facilitator Lonnie
Large increased from $4,740.00 to $15,000 while remaining a
f I i~ part time position because VIP doubled our services (adding
~ ~ a men's group in Kau to the existing Puna group) and

~ o; eliminated the community education position. Facilitators
_ w tr~~~~ ~ are paid on a per group basis.
~ j y~ * Table 2; Total Program Budget FY 98-99 and 99-00, Employee
~ Benefits/Payroll Taxes: The Violence Intervention Project
staff subcontracts with Bay Clinic and are responsible for
~ their own benefit, FICA etc. Contractors meet state
requirements and are issues IRS 1099's.
* Table 3, 7. Insurance, General Liability was left blank and
the correct amount ($2,459) inserted, with resulting totals
corrected on Table 3 and Table 5.
* Table 6; VIP is pursuing State and Federal as a member of
the Domestic Violence Inter-Agency Team under the Rx for
Abuse Act which funds medical providers as Domestic Violence
is a leading cause of injury to women.
