Re: Resolution Accepting Donation
Enclosed is a resolution accepting the donation of software licenses for the use of Street
Sense, and for Street Analytics, purchased for the Hawaii Fire Department by the Daniel
Sayre Memorial Foundation, P.O. Box 1285, Kailua-Kona, Hawaii 96745. This
software, with a value of $14,000 will be used in the Hawaii Fire Department's efforts to
identify, track and case -manage patients at risk of falling, people who are chronically
homeless, and those who utilize the EMS system to a high degree.

RESOLUTION AUTHORIZING THE ACCEPTANCE OF
A DONATION OF STREET SENSE AND STREET ANALYTICS LICENSES FOR USE BY
THE HAWAII FIRE DEPARTMENT
Pursuant to Section 2(g) of Rule 4 of the Rules of Procedure of the Council of the County of
Hawaii, this written request is submitted with my approval that the above -referenced matter be
waived from the Committee on Finance to the full Council for immediate action. In reviewing
this matter, timely approval is crucial. It is therefore advantageous that approval is granted and
the matter be placed onto the next Council agenda for review. However, in the event this request
is denied, for whatever reason, I understand the matter shall be referred to the Committee on
Finance for placement on its future agenda.

B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED):
The donation is given to the Hawaii Fire Department EMS Bureau.
The program called Street Sense is a real-time analytics tool that is able to identify, track and case manage
patients at risk of falling, chronic homeless and high utilizers of the EMS system. Street Sense is an
invaluable tool to help connect the most vulnerable members of the community with resources more
appropriate than EMS and subsequently transition them out of the 9-1-1 system. Street Sense monitors the
system, and when a target patient enters the 9-1-1 system, it sets off an alert. Most 911 public safety
departments are plagued by the challenge of what to do with a small number of individuals who demand a
significantly disproportionate amount of resources and who often cannot pay for them. Identifying these
people with Street Sense software and being able to intervene in real-time and route them to proper care
results in tremendous savings in terms of costs and unit hours, as well as reduces the emotional toll it takes
on crews responding to these individuals for non -emergent situations again and again.

The signing quote will establish the basic terms for the Deployment of Street Sense and Street Analytics at Hawaii Fire
Department. The terms contained in this quote are not comprehensive and it is expected that additional terms may be added.
and existing terms may be changed or deleted. The basic terms are as follows:

Total Interface Fees $14,000 one-time
Additional Terms
2) The following additional terms further describe the assumptions of this agreement:
a) Payment for Interfaces fees is due upon signing of this Purchase Agreement.
b) If Parties choose to end use of Street Sense/Street Analytics within 90 days from the Closing Date, all one-time
fees will be refunded.
Pricing is subject to change if the terms outlined are modified upon execution of the Software License Agreement. The
