Please explain the impact of these funds (previously received and/or projected):
• What are the program outcome measures? (How do we know the
project/program was/will be successful at meeting the identified
issue/need?) Please DO NOT give outputs.
• What is your sustainability plan to continue the program/project beyond
the grant funding cycle?
Recognizing there are so many needs in our community, how is this
program/project meeting a strategic issue? Please be specific and provide data for
the following:
• What will happen if we don't act now?
• How is the issue currently being addressed?
• Who are you partnering with to bridge the gaps and leverage resources?
• Who is being or will be affected?
• What are the consequences of not addressing the issue?
