Tuesday, April 7, 2020, the Council of the County of Hawaii will be considering a resolution in support of maintaining all four Medicaid plans on neighbor islands. I am writing this letter in support of testimony in support of this resolution. My interest in this proposal lies in the fact that I am a patient at the West Hawaii Community Health Center,the current Chairperson of its Board of Directors and a career healthcare financial service professional. One of the basic tenets in support of any Medicaid Managed Care program, such as Hawaii's Quest Program, choice of plan on the part of participants has been an important factor is providing cost-effective and quality patient tare. In my thirteen years of professional experience as the Finance Director of a program in the State of Rhode island that received Federal approval in 1994, the same year and under the same waiver as Hawaii's Quest Program, I am well aware of the problems that occur when a limited choice of health plan is given to recipients. In a change from four to only two plans in short period of time, recipients are faced with difficult choices. These choices include potential loss of access to their customary primary care doctors, changes in benefits based on plans' ability to offer optional services at their ownexpense and a "rush" to enroll in plans that provide better customer services. The result of this is confusion among participants and failure to use services correctly. From the provider's perspective, not all plans are equally attentive in prompt payment of claims, resulting in some physicians refusing reimbursement from those plans that result in higher amounts of uncollectible medical claims.
