NOTES:
1. This detail is intended for two—lane, two—way roadways with maximum speed limit of 25 mph. For other
roadway classifications, additional warning signs may be required. Refer to latest edition of the MUTCD.
2. For end of road(s) with access to side streets at 90't to the right or left, use appropriate sign in lieu
of 18"x18" diamond, (at center line of approach lane location), such as W1-6 or W1-7.
3. Use appropriate advance warning signs per the latest edition of the MUTCD.
4. Increase height of the end of roadway markers when the approach is on a vertical curve that obstructs
visibility of the markers. Desirable minimum sight distance for the end of roadway marker should be at
least 500'.
5. The barricade and signs should be placed at approximately 90' to the roadway to provide maximum visibility.
High intensity sheeting may be directional and must be oriented accordingly.

NOTES
1 No driveway approach shall interfere with municipal
facilities or other street structures.
2. No driveway approach shall interfere with the proper
runoff of surface waters into, or passage of waters
through existing drainage culverts, swales, ditches,
watercourses, defiles, or depressions. When in the
construction of a driveway approach, the proper
runoff of surface waters and other waters require
the construction of a drainage structure other than
a swale, such drainage structure shall be designed
by an Engineer and subject to the approval of the
Director.

The Contractor shall provide temporary erection bracing and shoring for all structural members
as required for structural stability of the structure during all phases of construction.
The concrete work shall conform to ACI 318-08 and shall attain a minimum compressive
strength of 4,000 P.S.I. at 28 days unless noted otherwise. Maximum aggregate size shall be
3/4 inch with a maximum slump of four (4) inches.
All Reinforcing Steel shall be Deformed Bars conforming to ASTM A-615, Grade 60. Bends
and Hooks shall be to Far Face of intersecting wall or slab.
Concrete cover for Reinforcing Bars shall be as follows, unless noted otherwise:
a Concrete cast against earth: 3"
b Concrete exposed to earth: 2"
c Concrete exposed to weather: 1"
Grates and Frames shall conform to ASTM A-36 and shall be Hot Dip Galvanized after
fabrication. According with ASTM A123.
The Concrete Adjustable Grating Seat shall be cast in place.
Top of Grating shall be set relative to the adjacent pavement/shoulder elevations.
The Drywell Intake Box is sized to allow for a maximum 36—inch Culvert connection. The
Designer must resize the Catch Basin as necessary. A minimum 8—inch wall thickness shall
be maintained at all times. See HDOT Std. Plan H-06 for Reinforcing around Culvert openings.
#2 Crushed Rock shall be manufactured from sound durable rock and shall be free from
organic matter and other deleterious substances. The wear when tested under AASHTO test
method T 96 shall not exceed 50 percent at 500 revolutions. Gradation (ASTM designation)
percent passing by weight shall be as follows:

The distance from the top of the precast portion of the Intake Box to the top of the Culvert
opening shall be a minimum of 14—inches.
Wall opening for connecting Culvert shall be constructed a maximum of 1-1/2 inch larger
than the Culvert outside diameter. Oversized and square openings shall not be allowed.
WELDING NOTES
1. All welds shall be 3/8" unless shown otherwise. Grind smooth, all exposed welds.
2. Design Loads:

1) Roadway shoulder: HS20
3. All Joints shall be fully welded.
