NOTICE OF NONPROFIT YEAR-END REPORT All recipients of the County of Hawaii Nonprofit Grants -In -Aid for FY 2022-2023 must submit a year-end report within sixty (60) calendar days after June 30 of the 2022-2023 fiscal year. DEADLINE FOR SUBMISSION: Submitted online via Google Forms no later than August 29, 2023, at 4:30 p.m. A link to the form can be found on the Department of Finance webpage, under the Budget tab as "Nonprofit Grant Forms". Partial reports will not be accepted, and there will be no further notice. REPORTING REQUIRED: To fulfill reporting requirements, you must provide as follows: • Program and contact information. • Total award amount and percentage of award claimed. • Updated Budget Form. • List program's activity outputs as outlined from your initial application and indicate measurement of accomplishment for each. Please explain if there is a more than 10% deviation +/-. • List program's short-term outcome measures as outlined in the Logic Model from the initial application and indicate a quantitative measurement of progress for each. Please explain if there is more than 10% deviation +/-. • In 1,500 characters, explain the progress of your program/project relative to your project timetable and describe next steps. Include identification of any issues or problems that may have impacted the development and implementation of the program/project. • A photo accompanied with a short narrative to illustrate progress towards meeting outcome measures and the public benefits derived from use of the County Grants - In -Aid funds.
