EnviroQuest, Inc. (EQI) is a privately held Hawaii corporation providing a comprehensive range of industrial hygiene, health, and safety services to public and private sector clients since 2001 with an average of 17 employees over the last 5 years. Situated in Aiea, Hawaii and Kanagawa, Japan, EQI services projects primarily in Hawaii, Japan, Singapore and Guam. Our clients include Hawaii public utilities, federal, state, and local governments, property management firms, commercial property owners, professional engineers and architects, law firms, contractors, and private homeowners. With local and international experience, EQI has emerged as one of the most experienced companies in Hawaii. We are committed to providing our clients with professional and responsive service. EnviroQuest, Inc. specializes in all aspects of industrial hygiene services. We conduct building surveys for hazardous materials (asbestos, lead, PCB's, etc.); develop specifications for the abatement of hazardous materials; provide continuous project management and air monitoring services during hazardous material abatement activities; conduct indoor air-quality investigations; measure workplace exposure hazards; and conduct employee awareness training classes for Occupational Safety and Health Administration (OSHA) and Hawaii Occupational Safety and Health compliance (HIOSH). EnviroQuest, Inc.'s main contact person is Steven Tanaka and his email address is stanaka@enviroquestinc.com. 2 Building Inspections for Hazardous Materials EQI staff routinely conducts building inspections for asbestos-containing material (ACM), lead-based paint (LBP), polychlorinated biphenyl's (PCB's), and mold infestation. The inspections are conducted by EQI industrial hygienists certified as an EPA AHERA Building Inspector and Management Planner and I or an EPA Lead Inspector and Risk Assessor. EQI staff have surveyed over 6,000 buildings in Hawaii, California, Japan, Singapore, Korea, Guam, Saipan, Kwajalein, and Diego Garcia. Hazardous Material Abatement Specifications EQI staff has designed over 3500 hazardous material abatement projects. Hazardous material abatement specifications are developed in accordance with Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA), Hawaii Occupational Safety and Health (HIOSH), and other regulatory agency requirements and guidelines. EQI works with our clients to develop creative, cost-effective solutions to addressing hazardous material problems, including public relations support and abatement project management. Abatement Project Management Project Management services are geared so that EQI manages the day-to-day tasks associated with the hazardous material abatement. EQI works closely with the building owner, contractor, and architect/engineer to ensure that every aspect of the abatement work is closely managed. Project management services include: • Air Monitoring • Baseline Samples • Work-in-Progress Samples • Post abatement Samples • Laboratory Analysis • Documentation Management 3 Air Monitoring Air monitoring is conducted during all phases of the abatement activity. The three sample types collected include: • Baseline Samples: Samples are collected to measure ambient material concentrations prior to any abatement action. • Work-in-Progress Samples: Samples are collected daily during the abatement activity. Samples are collected inside the work area/ containment, outside the work area/containment, and at the exhaust to any negative air filtration units. These samples will enable EQI to determine if work practices are contributing to an increase in material concentrations. If material concentrations become elevated, EQI will instruct the contractor to modify their work practices. • Post Abatement Samples: Samples are collected at the completion of each abatement action to ensure a hazardous material free area. Laboratory Analysis EQI utilizes only laboratories deemed proficient through successful participation in American Industrial Hygiene Association (AIHA) Proficiency Analytical Testing (PAT) and Environmental Lead Laboratory Accreditation Programs (ELLAP) as well as the U.S. Department of Commerce, National Institute of Standards and Technologies (NIST) National Voluntary Laboratory Accreditation Program (NVLAP). Indoor Air Quality Studies - Mold Remediation Indoor Air Quality (IAQ) is a rapidly evolving aspect of industrial hygiene. IAQ studies are conducted in accordance with current guidelines and recommendations by the American Conference of Governmental Industrial Hygienists (ACGIH), American Society of Heating, Refrigerating and Air-Conditioning Engineers, Inc. (ASHRAE), National Air Duct Cleaners Association (NADCA), and OSHA. EQI begins all IAQ studies by interviewing building occupants, evaluating the working environment, and then recommending appropriate corrective actions or sampling schemes. Sampling typically includes the collection of non-culturable spore samples using spore traps; culturable mold and bacteria samples using Andersen samplers; organic and inorganic vapors using sorbent tubes and collection bags. 4 BIOGRAPHIES As a service organization, our most valuable assets are the team of professionals dedicated to providing responsive and professional service to our clients. An overview of professional experience of our key personnel is provided herein. 5 Steven Tanaka - Principal/Industrial Hygienist Mr. Tanaka is an industrial hygienist with over 36 years of experience in industrial hygiene and environmental consulting. He has experience in all aspects of asbestos, lead, PCB, and indoor air quality assessments and abatement design; OSHA compliance air monitoring and workplace assessments; emergency spill response, containment and remediation; and providing OSHA and EPA compliance training. Mr. Tanaka oversees all the activities of EQI Hawaii and Japan offices, including daily operations, business development, and the technical development of the staff. Mr. Tanaka is certified as an instructor of EPA AHERA Asbestos MAP courses and EPA Lead courses. Mr. Tanaka has a working knowledge of Environmental Protection Agency, Occupational Safety and Health Administration and Japan Environmental Governing Standards in Japan, Hawaii, and California. Mr. Tanaka has worked throughout Japan and the Western United States. His educational foundation is in biology, chemistry and business.
