Please address the following in a revised application: 1. Include within the project description: Timing of construction phases, with dates to commence and complete construction; Reason for requesting phasing; Hours of operation for each proposed use; Max number of participants for each use; Number of employees. 2. Project Components section of application refers to building a single-story residential structure into a Church and notes the church will take up no more than 4,069 square feet of the dwelling. Who will occupy the residence and how does the residence fit into the construction phases?The site plans provided do not appear to include a dwelling component—please revise as necessary. 3. South Kohala Community Development Plan (SI<CDP)—The application has incorrect information about the CDP. The narrative needs to include a discussion on the SKCDP addressing how the proposed use permit is in alignment with goals & policies of the SKCDP. 4. The property is not within the SMA, nor is the entire island within the SMA. Please revise the section to state correct information regarding the SMA. S. Land Study Bureau classification section—incorrect classification rating. Provide correct classification and a discussion that pertains to the specifics of the subject property. 6. Access road &traffic section: Provide description of road, width and condition of pavement; right of way dimensions; distance from main highway.Also missing are detailed descriptions of traffic effects and mitigations. The application states intermittent evening sessions will average 0 vehicular movements? Please explain in more detail how many participants and vehicles are estimated for each type of use, including evening sessions. 7. Infrastructure issues: a. Water: Submit the letter from Hawaii Water Service verifying there is adequate water supply for the proposed project. b. Wastewater: The application &civil site plan refers to both septic tanks and sewer lines. Please clarify if both are being installed? Is the project connecting to an existing sewer system? 8. Elevations of proposed structures: As stated on the use permit application, provide elevations of all proposed structures. Please submit a new, revised version of the complete application, with site plans and any other drawings, to replace the old version. We will hold the filing fee, and the new application can be uploaded to the same EPIC case file: PL-USE- 2023-000018 Please contact me with any questions.
