After a careful review of the above referenced Use Permit application,we are returning the application as incomplete
and requesting the following information be provided:
1. Please review the enclosed Use Permit application and address each question separately and specifically.
2. Under item 3A, please provide a detailed written description of the proposed use, a statement of objectives and
reasons for the request, including the square footage of the proposed structures. The times of each proposed
use will be required,which includes but is not limited to the church services as well as the after-school children's
programs,tutoring and supervised safe locations for the children of the nearby schools that is discussed further
in the application. Also include a detailed description of the two phases and when each phase is expected to
commence construction and be completed. Does the applicant currently operate their services from another
location? If so, please include in your project objective.
3. Under item 3B, please provide a description of the subject property in sufficient detail to precisely locate the
property. Describe existing uses, structures, and topography on the existing property. If portion of property is to
be used, state the use of remainder of property.
4. Under item 3C,the following information should be added as they are land use designations that affect the
application:
a. The State Land Use Designation of the subject property is Urban.
b. County Zoning is Single Family Residential-10,000 square feet(RS-10)
c. The General Plan Land Use Pattern Allocation Guide (LUPAG) map designates the site as Low Density
Urban.
d. The South Kohala Community Development affects the property.
e. The property is not within the Special Management Area (SMA). Please list how far the subject property
is from the shoreline.
5. Under item 3D, please identify and discuss the surrounding uses of the neighboring parcels (i.e.,types of
businesses, office, park use, residential uses, etc.)
6. Under item 3F, 3G, 3H, please provide information regarding any archaeological, historical and/or cultural
resources on the subject property and floral and faunal resources, if any.
a. Archaeological. Is there a previous archaeological study that was done for the site? Why was no
archaeological study completed for the proposed project?
b. Floral and Faunal. What was the site previously used as? What types of vegetation are located on the
subject property? Which vegetation are considered endangered? In terms of faunal,what types of
faunal have been located on the subject property? Of the faunal,which faunal are listed as endangered
species? How often are these species located on the subject property? What types of impacts will the
proposed project have on the floral and faunal resources?
c. Cultural Resources.The Hawai'i State Supreme Court's "PASH" and "Ka Pa'akai 0 Ka 'Aina" decisions
require decision-makers to consider a project's impact to native Hawaiian gathering and fishing rights.
Specifically,there must be a discussion of the cultural, historical, and natural resources and associated
traditional and customary practices of this site and the impact of this project to these resources and
practices.
d. Climate, Soil and Topography. Please provide a discussion on the Land Study Bureau Overall Master
Productivity Rating for the subject property as well as the Agriculture Lands of Importance to the State
of Hawaii classification of the subject property.

7. Under item 31, please provide information regarding the access road(s)to the subject property, the description
of the road (e.g.,width,type of surface and condition of the roadway) and the right-of-way width. Describe how
far the proposed request is from the main highway as well as information on whether these are State or County
owned and maintained roads or if they are private roads. If they are private, please submit evidence of access
rights. Please summarize existing traffic conditions, and anticipated increases in traffic and possible traffic
impacts from proposed use and any recommended improvements, if any. Discuss current or proposed driveways
to the subject property. How many trips will be generated by the proposed use during the AM and the PM?
Please note that a formal study may be requested by the Department of Public Works or Department of
transportation during the review process.
8. Under item 4A, please summarize how the granting of the proposed use shall be consistent with the general
purpose of the zones district,the intent and purpose of the Zoning Code,the County General Plan, and the
South Kohala Community Development Plan (CDP).
a. You will need to detail how the requirement for parking will be met. The Zoning Code currently requires
one (1) stall for every 75 square feet of sanctuary space. For the potential classroom space, one (1) stall
is required for every 20 students. (You can find a copy of the zoning code here:

b. What goals and policies are the proposed request consistent with in the General Plan. (You can find a

c. Does the South Kohala Community Development Plan support the proposed use? How does the South
Kohala Community Development Plan support the proposed use? (You can find the South Kohala

planning/cdp/skohala/doc)
9. Under item 4B, please provide more discussion on how the granting of the proposed use shall not be materially
detrimental to the public welfare nor cause substantial adverse impact to the community's character or to
surrounding properties.
10. Under item 4C, please provide more discussion on how the granting of the proposed use shall not unreasonably
burden public agencies to provide roads and streets, sewers,water, drainage, school improvements, police and
fire protection and other related infrastructure.
a. Water. Discuss the anticipated water use that will be necessary for the proposed use. Please submit the
letter from Hawaii Water Service verifying that there will be enough water to serve the proposed
project.
b. Wastewater. Please provide a discussion on what type of wastewater system will be installed for the
proposed project. Please include the system on the site plan.
c. Solid Waste. Discuss how the solid waste be handled.
d. Police and Fire. Discuss how far the subject property is the police,fire, and hospital services.
Based on the preceding information,the applicant's Use Permit Application is hereby returned and as such, we will be
returning your filing fee of$50.00.. When the applicant re-submits, a complete Use Permit Application and all
requested/required information and the appropriate filing fee will be required at that time.
