Dear Mr. Kanuha:
Bureau Veritas North America, Inc. (Bureau Veritas) has been retained by Cellco Partnership and its
affiliates doing business as Verizon Wireless (Verizon Wireless [VZW])to conduct a site evaluation for the
Federal Communications Commission (FCC)of the above-referenced proposed communications facility
project.
Pursuant to FCC rules regarding review under Section 106 of the National Historic Preservation Act
(NHPA), we are contacting you for information with regards to any potential impacts to historic properties,
religious, or cultural resources that the proposed wireless telecommunications facility may have at the
site.
Per FCC requirements, Bureau Veritas is also contacting other applicable agencies and native Hawaiian
organizations and will be posting a Legal Notice in the local newspaper as a public notification effort. A
formal submission and application for zoning approval of the proposed project is being handled by other
professionals under separate project tasks.
Project Description
Site name/location: The proposed VZW HI3 Paniolo communications facility is located at 68-3670
Paniolo Avenue, Waikoloa, Hawaii County, Hawaii 96738.
Latitude/Longitude Coordinates: 19°56' 37.54" North, 155°47' 29.07"West
Legal Description: The subject parcel is described as the parcel of land designated as TMK Number: (3)
6-8-002: Parcel 031. According to the County of Hawaii Real Property Tax Office, the county zoning
designation for the subject parcel is"Residential."The subject property, identified as the VZW HI3 Paniolo
communications facility, is located in an undeveloped area in an open field located north of the church
building on the subject parcel.

Property Owner: Waikoloa Baptist Church currently owns the site.
Current Land Use: The subject parcel is currently used as the Waikoloa Baptist Church and residence
Proposed Action: According to VZW, the proposed action involves the leasing of an approximate 25-foot
by 30-foot lease area in an open field north of the Waikoloa Baptist Church, for the installation of a 65-foot
monopalm (monopole disguised as a palm tree) mounted with 12 panel antennas. Associated equipment
cabinets, and a 30 kilowatt generator are also planned to be located within the lease space.
Construction activity is anticipated to have a minor impact to the ground surface.
Adjoining Property Use: The adjoining properties consist of residential lands. The adjoining properties
are described below:
• North: Undeveloped land
• East: Paniolo Avenue, beyond which are residential properties
• South: Waikoloa Baptist Church
• West: Undeveloped land
Figures and photographs of the subject property and surrounding area are attached.
Information Request
As noted, we are requesting information on any native Hawaiian cultural, religious, and/or burial sites;
historic places, artifacts, and/or other related concerns at the proposed project site; and whether or not
any of these issues will be impacted by the proposed communications facility project.
