Okarnoto, Maiiagei-Cliiiet'l--'t,agiiiecr <br />SU BJECT: Special Management Area Use Permit Application (PL-S.MA-2024-000060) <br />Applicant: Niel.layusinghe <br />Request: To Develop an Fo"r (4) Story, Seventeen (17) Unit Apartment Building <br />Tax Map Key 2-1-014:002 <br />We have reviewed the subject application and have the following cornnients and conditions. <br />Water can be rnade available from ail existing 6 -inch waterline within Kalaniana'ole Avenue fronting the subject <br />parcel in accordance with the Department's existing water availability conditions, which are subject to change <br />without notice. T'here is an existing service lateral installed to the parcel capable of acconi nlodating a '/8 -inch <br />meter, which is limited to an average usage of 400 gallons per day. <br />The F)cpartnient has no objection to the proposed application, subject to the applicant understanding and <br />accepting the l'ollowing conditions: <br />The Department requests that the applicant submit estimated nlaxiniuni daily water usage calculations <br />for the proposed development, for review and approval. The water usage calculations shall be prepare(] <br />by as professional engineer licensed in the State of I lawai'i and should include the estimated peak flow <br />in gallons per minute (GPM) and the total estimated maxinlurn daily water usage in gallons per day, <br />including all irrigation/landscaping use. <br />Upon receipt of the water usage calculations above, the Depailtlient will make a determination as to the <br />water commitment deposit amount and prevailing facilities charge (subject to change) to be paid, if <br />necessary. Based oil the water deniand calculations, the Department will deterrnine the appropriate <br />service lateral and meter size required for the proposed developnlent. <br />The proposed use wild require the installation of a reduced pressure tyl)C backflow prevention assenibly, <br />within five (5) feet ofthe meter oil private property. I I'a larger or additional meter is required, a <br />backflow prevention assembly will also be required for the that meter.
