Stewardship Grant Application Checklist
1. Application Submission
Has the applicant provided the items listed below?
❑ 1.Application Form: Submitted a signed, completed form provided by the Department of
Finance for the specific project. (Charter 10-16(h)(4)(A))
❑ 2. IRS 501(c)(3) Determination Letter: Confirming the organization's 501(c)(3) status or
that of an umbrella organization. (Charter 10-16(h)(4)(13))
❑ 3. Bylaws and Mission Statement: Copy of the organization's bylaws and mission
statement. (Charter 10-16(h)(4)(C))
❑ 4. Detailed Business Plan: Information provided in the application contains a business
plan detailing the project,including timeframes,costs,activities, and any additional
information requested by the Department of Finance. (Charter 10-16 (h)(4)(D))
❑ 5. Signed Agreement for Reporting: Submitted a signed agreement to file a report within
one year after receiving funds or 30 days following project completion.The applicant
acknowledges that the report should include project details, expense receipts (proof of
payment) and any other information requested by the Department of Finance. (Charter 10-
16(h)(4)(E))
❑ 6.Agreement for Non-Salaried Officers and Board Members: Submitted a signed
statement confirming that no officers,board members, or employees will receive a salary
from the funds for general duties,with the exception of compensation for specific duties
outlined in the business plan. (Charter 10-16(h)(6))
❑ 7. Conflict of Interest Disclosure Form: Disclose any potential conflicts of interest
between your organization's officers,board members, or employees and the County
Council, Mayor, Managing Director, or other specified officials.- Sign Attached Form. (HCC
2-137(4))

11. Grantee Qualifications
Has the applicant provided sufficient evidence, documentation, and/or statements
confirming these requirements?
❑ 8. Is the organization able to complete the project on time and within cost estimates?

❑ 9. Hawaii State Authorization: Is the organization authorized to conduct charitable work
in Hawai'i? (HCC 2-137(3))
❑ 10. Benefit to Hawai'i County: The organization's purpose is to provide benefits to the
people of Hawai'i County. (HCC 2-138(1))
❑ 11. Governing Board: Do the organization's board members serve without compensation
and adhere to a written conflict of interest policy which complies with requirements of the
Internal Revenue Service, State Law,and rules and regulations of the finance director? (HCC
2-137(1))
❑ 12. Bylaws and Policies: Do the bylaws or policies describe the conduct of business,
including management,audit, fiscal procedures, and policies on nepotism and conflict of
interest? (Nepotism defined: HCC 2-136(1); Conflict of interest defined: HCC 2-136(4))

❑ 13. Experience or Expertise: Does the organization have at least one year of experience or
sufficient expertise to successfully complete the project? (HCC 2-138(2))
❑ 14. Licensing and Accreditation: If applicable,is the organization licensed and accredited
as required by law? (HCC 2-137(3))
❑ 15. Compliance with HCC 2-138: Confirm compliance with HCC 2-138,including merit-
based employment,nondiscrimination, proper use of funds,and access for oversight. (HCC
2-138 &2-136(6))
