(TMK) (Project address) Print Name: Signature: (Property Owner or Licensed General Contractor) (Property Owner or Licensed General Contractor) Submittal Instructions: 1.To start a transfer, please visit the Building Division Homepage, click Request EPIC Assistance, complete allrequired fields, then select “Transferring a permit or licensed professional”. 2.You will receive an email response from cohbuild@hawaiicounty.gov to let you know when your permit is open foryou to upload your transfer documents.3.After you receive the email, log into your Hawaiʻi County EPIC account and upload your completed transfer formand the new contractor’s Project Declaration form to your permit case.4.Staff will process your transfer in the order your forms are received.5.You will receive an email notification from NoReplyEPIC@hawaiicounty.gov to alert you that the transfer fee isready to be paid. ver. 01/13/2025 CHAPTER 5 CONSTRUCTION ADMINISTRATIVE CODE Section 5-5-3(c). Designation of person, contractor, or subcontractor who will do work. (c) If subsequent to the issuance of a permit, a homeowner either ceases to employ the owner-builder exemption and engages a contractor for any phase of work, or there is a change in the designation of any contractor for any phase of work, the permit holder shall file a written request to the authority having jurisdiction for approval of these changes. The request shall include: the change in designation; a revised declaration form for each contractor or subcontractor engaged to do electrical or plumbing work upon the building, structure, or project; and a non-refundable fee of $100.
