Pursuant to Rule 3-2-D-12 and 6-1-D, of the Rules and Regulations of the Liquor Commission of the County of Hawai’i, application is hereby submitted for a Teen Event Permit. (Multiple events scheduled within a one-month
period may be included on one application.)

Your application for a Teen Event Permit is approved and is subject to the following conditions:
1.Liquors shall not be consumed in the area set aside for use for this event.2.All liquors shall be removed or locked from the area set aside for use for this event.
3. Entertainment, games and conduct of participants shall be in compliance with the liquor laws.4.The licensee shall ensure compliance with curfew laws applicable to minors.
