AND NOTIFICATION RECORD
PL-USE-2026-000042 Development of a Rural Outpatient Behavioral Health Clinic

Reporting Period: April 18, 2026 – June 5, 2026
PURPOSE
This Supplemental Community Engagement, Public Outreach, and Notification Record is
submitted to document community engagement, public outreach, notification activities,
and follow-up actions occurring after April 1, 2026, following submission of the Applicant
Community Notification and Outreach Timeline previously included within the Windward
Planning Commission record.
April 18, 2026 – Elected Official Outreach
Applicant initiated outreach to Councilmember Heather Kimball regarding the proposed
Big Island Neuro Health Foundation outpatient behavioral health clinic project in
Pepeʻekeo.
The correspondence introduced the project, provided an overview of the proposed rural
outpatient behavioral health clinic, described the project's role in expanding access to
specialty behavioral health services in East Hawaiʻi, and requested an opportunity to
discuss the project further.

Applicant met with Councilmember Heather Kimball to introduce the proposed outpatient
behavioral health clinic project and answer questions regarding the project and Planning
Department review process.
Applicant subsequently transmitted follow-up correspondence thanking Councilmember
Kimball for meeting and reiterating availability to answer community questions.

May 17, 2026 – Community Presentation
Applicant attended the scheduled Pepeʻekeo Community Development Corporation
(PCDC) Community Meeting and presented information regarding the proposed outpatient
behavioral health clinic.

• Project purpose
• Community need
• Proposed services
• Operational characteristics
• Anticipated patient volume
• Traffic expectations
• Community impacts
• Questions and answers
Applicant remained available following the presentation to answer additional questions
from attendees.

May 17, 2026 – Notification Boundary Clarification
During community discussions, questions were raised regarding whether the property
located at 28-1142 Old Mamalahoa Highway should have received formal notification.
Applicant independently reviewed the Hawaiʻi County Real Property Tax Office GIS
mapping system and utilized the County GIS buffer tool to verify the required notification
perimeter.
The review indicated that the referenced property appeared outside the required 300-foot
notification boundary measured from the subject property.
Applicant subsequently provided clarification and supporting GIS documentation to
Councilmember Heather Kimball and representatives of the Pepeʻekeo Community
Development Corporation and requested that the clarification be shared with the affected
property owner.

Applicant provided courtesy notification to the Pepeʻekeo Community Development
Corporation (PCDC) advising that the required notification mailing had been completed.
Copies of the hearing notification letter and Windward Planning Commission agenda were
provided to PCDC.
This communication was voluntary and was not required by the Planning Department.

May 22, 2026 – EPIC Filing
The Affidavit of Mailing documenting the notification mailing was executed, notarized, and
uploaded to the County of Hawaiʻi EPIC system.
The filing provided public documentation confirming distribution of hearing information
and notification materials.
June 5, 2026 – Windward Planning Commission Public Hearing
Use Permit Application PL-USE-2026-000042 scheduled for public hearing before the
Windward Planning Commission on June 5, 2026.
SUMMARY
This Supplemental Community Engagement, Public Outreach, and Notification Record is
intended to complement the Applicant Community Notification and Outreach Timeline
previously submitted to the Windward Planning Commission record, which documented
community outreach activities beginning February 3, 2026, and continuing through April 1,
2026.
Between April 18, 2026, and the scheduled Windward Planning Commission public hearing
on June 5, 2026, Big Island Neuro Health Foundation continued its community
engagement, public outreach, and transparency efforts related to the proposed outpatient
behavioral health clinic in Pepeʻekeo. During this period, outreach was conducted with
elected officials, project information was presented at the Pepeʻekeo Community
Development Corporation community meeting, follow-up communications were provided
to community stakeholders, and public documentation was filed through the County's
EPIC system. When questions arose regarding notification boundaries, the Applicant
conducted an independent review and provided clarification supported by GIS mapping
documentation.
This supplemental record is submitted to document outreach and engagement activities
occurring after April 1, 2026, and demonstrates the Applicant's continued commitment to
transparency, public participation, and community engagement throughout the permit
review process.

My name is Jessica Stevens, and I serve as the Chief Executive Officer of the Big
Island Neuro Health Foundation, a 501(c)(3) organization focused on expanding access
to specialty outpatient behavioral health services in rural East Hawaiʻi.
We are currently advancing a capital project in Pepeʻekeo to develop a community-
based outpatient clinic that will provide much-needed access to evidence-based mental
health services, including Transcranial Magnetic Stimulation (TMS). This project is
designed to directly address significant access gaps across East Hawaiʻi County, where
geographic and workforce limitations continue to impact timely care.
The project is being structured in alignment with the United States Department of
Agriculture Community Facilities Direct Loan Program and represents an approximate
$6 million investment in rural healthcare infrastructure. As we develop a comprehensive
capital stack, we are seeking to engage with Hawaiʻi Community Foundation to better
understand potential opportunities for partnership.
Specifically, we welcome the opportunity to discuss available support for rural health
access and behavioral health infrastructure projects of this scale.

Aloha Councilmember Kimball and Pepeʻekeo Community Development Committee,
I would like to clarify the surrounding property owner notification process associated
with the proposed Use Permit application for the Big Island Neuro Health Foundation
outpatient behavioral health clinic project located at 28-1167 Old Mamalahoa
Highway, Pepeʻekeo, Hawaiʻi (TMK: (3) 2-8-007:084).
During the recent PCDC meeting on May 17, 2026, the property owner stated that
Councilmember Kimball had confirmed that the property associated with the last name
BELLO, located at 28-1142 Old Mamalahoa Highway, was within the required 300-foot
notification perimeter and therefore should have received formal notification.
Following that discussion, we reviewed the Hawaiʻi County Real Property Tax
Assessor/GIS website again, utilizing the County GIS buffer tool. The buffer was
measured from the subject property's perimeter boundary, consistent with Hawaiʻi
County Planning Department notification requirements for Use Permit applications.
Based on the GIS buffer analysis, the BELLO property located at 28-1142 Old
Mamalahoa Highway appears to fall outside the mandatory 300-foot perimeter
notification boundary measured from the subject property's perimeter. As a result, the
property was not included within the formal notification list generated for the required
mailing.
For transparency purposes, the GIS perimeter image utilized during the review process
is attached for reference.
Additionally, I respectfully request that clarification also be provided directly to the
property owner associated with the BELLO property at 28-1142 Old Mamalahoa
Highway: based on the Hawaiʻi County GIS perimeter buffer review, the property was
determined to be outside the required 300-foot notification boundary for the subject
application. This clarification may help prevent any further misunderstanding regarding
the formal notification requirements associated with the Use Permit process.
