LISTING OF RESPONSIBLE ENTITIES

Title 3, Subtitle 11, Chapter 22, Subchapter 5, Section 3-122-21(8), the Bidder is required to list below the name of
each person or firm to be engaged by the Bidder as a joint contractor or subcontractor in the performance of the
contract. The Bidder must also list the nature and scope of the work to be performed by each joint contractor and
subcontractor.
Furthermore,
shall provide the corresponding contractor license identification number issued by the State for each entity listed.
