BACKGROUND INFORMATION Proposed use statement of oh'ectives reasons for the re guest The proposed use of the subject property is to provide a small private medical clinic to Pahoa and the surrounding neighborhoods. This family and community health center will provide personalized proactive patient driven care by a multidisciplinary team. The team will include, but not be limited to, two Family Nurse Practitioners collaborating with a SID, a certified Dietitian, and a Behavioral Health Specialist. The objective will be to offer a private health center within the Pahoa Village community that will offer robust primary care, attending to acute and chronic disease management with a strong focus on the overall health and wellness of the community. The reason for this request is to allow the use of this residential property for operating a small health center. There are many businesses in the immediate area, but most are offering food and drink. There is one existing community health center, Pahoa Family Health Center that offers medical services. with the recent growth of the surrounding community, there is a substantial need for health care providers who offer quality health care services. The proposed hours of operation will be Monday through Friday from 7 am to 5 pm. The clinic will open every other Saturday from 8 am to 12 noon. The center will start with 2 Family Nurse Practitioners as Primary Care Providers. There - will one Registered Nurse and 1 receptionist. The clients will number from 2 to 3 per provider per hour. That would total 4 to 6 clients per hour for the clinic. 0 There will be on -site parking provided on the west side of the property with a minimum of seven (7) stalls (Sec. 25- 4-50). Handicap stall and loading area will be provided in the attached carport. A handicap walkway and ramp will lead from the parking stall to the interior of the center. Description of the RrORerty The subject property contains 1.31 acres of land area. The existing residence has 2037 sq. ft. of living area and a 420 sq. ft. carport. The original year of construction was 1952. The exterior walls are hollow tilt and the interior is double wall construction. The property sits on the exact opposite side of Pahoa village Road from the Star of the Sea Catholic Church. Immediately west of the property is Paul's Repair Service which is a gas station and automotive repair center. Just past Paul's repair to the west are the Luquin's Mexican Restaurant building and other retail businesses like Lola's Boutique, Pahoa Puna Buy and Sell, and a Yoga center. On the East side of the property is another residential property which shares a common wide, expansive front lawn. This provides a lot of "green" space between the main government road and the buildings. Then farther to the East lies the Kamehameha Early Education Program (pre - school), Pahoa Elementary School with the adjoining County of Hawaii park and gyre, and Pahoa High and Intermediate School. Generally speaking, there is commercial activity to the west of the property, one residence and mostly educational institutions to the east of the property, religious institution to the north (across the street) of the property, and vacant land and housing to the south of the property. The property sits on one of the highest points of Pahoa town and dews to the west include Pahoa town, the County Recreation Center, with views of Mauna Kea and Mauna Loa on clear days. The property slopes down to the Pahoa village roadway, but about two thirds of the property is level from the house site to the north side of the property. There is an existing 100 foot asphalt paved driveway from the main road to the house. Landscaping is predominantly on the west boundary of the property with a grassed lawn fronting the Pahoa village Road. Iescri ntion_ access Access to property is off Pahoa Village Road. Pahoaa Village Road is a two lane pave roadway. Existing driveway is asphalt paved and about twelve feet in width. I* • Traffic Imaacts r� Property is located near four schools: Iarnehameha Early Education Program (DEEP), Pahoa Elementary School, Pahoa High and Intermediate School, and Pahoa Montessori Pre - School and Kindergarten (located on Catholic Church property). There is a school zone speed limit of 25 mph. when school is in session, traffic is increased in the morning before- school hours and in the afternoon in the after school hours. Because the patient load will be up to 6 per hour and because they will be arriving and departing at different tunes, there will be minimal impact on traffic entering and leaving the property from Pahoa village Road. Avallahlli of Utilities County water is installed. Telephone and cable is available and will be installed. Sewage disposal is currently by cesspool, but septic system will be installed as condition of Use Permit. II. REQUEST CONSISTENT WITH STANDARDS Zonm* and General Plan The property is in Pahoa village town on the edge of a commercially zoned district. The State land use designation is urban. The County General Plan designation is medium density urban. Both designations allow for a project like this proposal. The current zoning for this property is RS 10. The zoning code allows hospitals, nursing homes, and other similar uses devoted -to the care or treatment of the aged, sick, or infirmed in the RS district provided a use permit is obtained for the use. Public welfare and Community impact 4DThe granting of the proposed use of this property as a small family health center will not be materially detrimental to the public welfare nor cause substantial adverse impact to the community's character or to surrounding properties. In fact, the proposed use will substantially be beneficial to the public welfare. .A. lot of people now drive to Hilo to get medical attention because the current clinic is often overbooked and appointments to see primary care providers are limited. It will not impact the community in a negative way, but positively supplement the amount of health care services we have now in our area. The one residential property in the area is right next door, and they have given their blessing to this project. A family representative will testify in the positive when the Planning Commission hearing occurs. PubUc agencies jMRact There will be no need for the State or County government to make any road improvements. The applicant will replace existing cesspool with a septic system to meet Hawaii Department of Health requirements.. County water is already in place. There is a fire hydrant fronting the property. Drainage 1s not a problem. The property does not sit in a Mood zone. Schools surround the property and there will be no impact on therm.
