The property is located on the mauka side of <br />Hawai‘i Belt Road, between Hawai‘i Belt Road and Old Māmalahoa Highway, <br />approximately 0.7 miles east of the Mud Lane–Hawai‘i Belt Road intersection and between <br />Mile Marker 51 and 52, Kapoaula, Kamoku, Hamakua, Hawai‘i, TMK: 4-7-007:042. <br /> <br /> <br />2.APPLICANT: MARK G. JERNIGAN (SPP 09-081) <br />Request to amend Condition No. 2 (life of permit) of Special Permit No. 09-000081, that was <br />originally approved to allow the removal of cinder on 43,608 square feet of land situated <br />within the State Land Use Agricultural District. Specifically, the amendment seeks a 5-year <br />extension to the life of the permit. The property is located along the south side of Mahimahi <br />Drive, approximately 356 feet east of the Mahimahi Drive–Lurline Lane intersection, <br />Hawaiian Ocean View Estates Subdivision, Kahuku, Ka’u, Hawai‘i, TMK: 9-2-145:59. <br /> <br /> <br />3.APPLICANT: RONALD B. QUADE, D.D.S. (SPP 14-168) <br />Application for a Special Permit to allow the establishment of a dental clinic within an <br />existing 4-bedroom single family dwelling located on a 1.131-acre parcel situated within the <br />State Land Use Agricultural District. The subject property is located along the west (mauka) <br />side of the Volcano Highway (Hwy 11), 1/3-mile south of Mile Marker 19 in Mt. View, <br />Puna, Hawai’i, TMK: 1-8-021: 029. <br /> <br /> <br />
