RE. Notary Public Commission Application for Dennis Kauka, Jr.
This office requests that Dennis Kauka, Jr , be designated a "government
notary" As our clerical services assistant at the Office of the Mayor, he will perform the
services of a government notary without charge His services as a government notary
includes documents pertaining to government boards, commissions and/or
departments.
As a public officer, Mr Kauka will assist other government agencies as needed
during normal business hours to notarize affidavits, contracts, memorandums of
understanding and the like There will be approximately 15-20 notarial acts performed
per month
