INSTALLATION ACKNOWLEDGEMENT

I hereby state: 1. This is for a residential, single family dwelling (not incl. townhouses, condos or apartments).
2. That I am a Hawaii licensed architect / civil engineer / structural engineer in good standing.
3. That this project was designed by me or under my direct supervision and follows general engineering practice.
4. That I am responsible for the observation of construction.
5. That the panels and associated equipment will be installed in accordance with manufacturers’
recommendations and industry standards.
6. There shall be a clearance of at least 1 foot around the plumbing vents and no modifications shall be made to these vents without obtaining a separate permit for these alterations.
7. The roof framing is of wood construction.
8. The mounting system is pre-manufactured.
9. The panels and inverter are not located in the eaves or overhangs.
10. The panels will not be located within 10” of the roof ridge.
11. The plans and specifications are complete and accurate to the best of my knowledge.
If any of the requirements are not met, the building permit shall be invalidated and subject to appropriate
action by the County of Hawaii.
By stamping and signing below, I acknowledge that:
1. All of the information I have provided in this form and permit application is true and accurate 2. I have been authorized by the owner to submit this permit application
3. I am responsible for ensuring compliance to all applicable codes 4. I hereby agree that any violation of the Building Code, Electrical Code, Plumbing Code and/or
Fire Code may result in the suspension or revocation of the building permit 5. I have read and understand the County of Hawaii’s building permit requirements
