RE: Rural Health Network Development Planning Program:
HRSA Grant 18-034
The County of Hawaii strongly supports Community First's grant application to
HRSA's Federal Office of Rural Health Policy Community-Based Division Program:
"Rural Health Network Development Planning."
We understand how important an organized and coordinated health network of
medical and social service organizations can be for delivering more effective care to those
with complex medical and social needs in East Hawaii, in particular the high cost and high
need patients.
Through the funding of this grant, we support the Community Action Network's
planning and implementation activities that include:
1. A common data and care coordination platform
2. The Accountable Health Communities Health Related Needs Screening Tool
3. A community QA committee for case studies across medical and social service
providers
4. Exception funding for critical, unmet needs.
Thank you for the opportunity to support this grant and we are excited to see this
project funded.
