We understand how important an organized and coordinated health network of medical and social service organizations can be for delivering more effective care to those with complex medical and social needs in East Hawaii, in particular the high cost and high need patients. Through the funding of this grant, we support the Community Action Network's planning and implementation activities that include: 1. A common data and care coordination platform 2. The Accountable Health Communities Health Related Needs Screening Tool 3. A community QA committee for case studies across medical and social service providers 4.
