1.0CERTIFICATIONS AND LIMITATIONS
Lehua Environmental Inc. has completed thissupplementallimited hazardous materials survey
for the County of Hawaii’s Civil Defense Warehouse located on630 E. Lanikaula Street in Hilo,
Hawaii. This report is a supplement to the April 2008 HazardousMaterials Survey for the
County Facilities report by EKNA Services, Inc. (Previous Report). LEI’s findings contained
herein are based on research, site observations, government regulations and laboratory data,
which were gathered at the time and location of the study.Opinions stated in this report do not
apply to changes that may have occurred after the services were performed.
LEI has performed specified services for this project with the degree of care, skill and diligence
ordinarily exercised by professional consultants performing the same or similar services. No
other warranty, guarantee, or representation, expressed or implied, is included or intended; unless
otherwise specifically agreed to in writing by both LEI and LEI’s Client.

2.0EXECUTIVE SUMMARY
Lehua Environmental Inc. (LEI) has completed this supplemental limited hazardous materials
survey for County of Hawaii’s Civil Defense Warehouselocated on 630 E. Lanikaula Street in
Hilo, Hawaii (Subject Site). This report supplements the Previous Reportconducted by EKNA
Service, Inc.On December16, 2014, LEI personnel performed site reconnaissance to identify
and inventory suspect asbestos-containing material (ACM) and lead painted building materials
not previously identified atthe Subject Site.
Summary of Asbestos-Containing Materials Survey
During this survey, LEI collected and analyzed three (3) samples of suspect asbestos-containing
material (ACM). None of the sampled materials containeddetectable concentrations of asbestos
by laboratory analysis. Table 1 located in Appendix I summarizes these results. Additionally,
photographs of the sampled materials are located in Asbestos Survey Photograph Login
Appendix II.
Summary of Lead PaintSurvey
LEI personnel inspected the Subject Site for previously unidentified lead painted materials.
None were observed; therefore no paint samples were collected.

3.0INTRODUCTION/PURPOSE
The purpose of this supplemental limited hazardous materialssurveywas to investigate the
Subject Sitefor the presence of hazardous materials not previously identified in the Previous
Reportfor the Subject Site.LEI identified the hazardous materials not previously identified in
the Previous Report to be suspectasbestos-containing materials (ACM). Specifically, LEI
completed the following tasks:
Reviewedthe 2008Hazardous Material Survey Reportfor the Subject Sitecompleted by
EKNA Services, Inc. (Previous Report);
Performed site reconnaissanceand visually inspected the areas ofthe Subject Site;
Collected samples of suspectACMfrom the Subject Site;
Submitted thesamples of suspectACMto NVL Laboratories, Inc.for analysis;and
Prepared this report documenting the field activitiesand the results of the survey,
including analytical results, photographs, recommendationsand maps.

4.0METHODOLOGY
4.1AsbestosSurvey
LEIpersonnel collected a total of three(3) samples of suspectbuilding materials for asbestos
analysis. All of the suspectACM samples were collected in general accordance with EPA
guidelines and recommendations.
The suspectACMswere wetted with amended water before sample collection. A small piece
was then carefully cut out and placed into a labeled re-sealable plastic bag. Thesampling
equipment was cleaned between each sample collection to avoid cross-contamination between
samples. The approximate quantity of each suspectACM was noted. Sample locations were
randomly selected in accordance with EPA protocols and recommendations.
All samples were properly logged and recorded following strict chain of custody procedure and
submitted to NVLLaboratories, Inc.in Seattle, Washingtonfor analysis by polarized light
microscopy in accordance with EPA Method 600/R-93/116. NVLLaboratories, Inc.is
accredited for bulk asbestos analysis through successful participation in the National Voluntary
Lab Accreditation Program (NVLAP).
4.2Lead Paint Survey
LEI personnel inspected the Subject Site for previously unidentified lead painted materials.
None were observed; therefore no paint samples were collected.

5.0RESULTS
5.1AsbestosSurvey
The Occupational Safety and Health Administration (OSHA) Asbestos General Industry and
Construction Standards define ACM as materials containing more than one percent asbestos (29
CFR 1910.1001 and 29 CFR 1926.1101, respectively).
Three(3)sampleswere collected and analyzed for this survey. Noneof the sampled materials
containeddetectable levelsof asbestosby laboratory analysis. As a result, nohomogenous ACM
wasidentified.Table 1located in Appendix I summarizes these results.
Photographs of sampled materialsare provided in Appendix II.Laboratory analytical reports
and chains of custodyare provided in Appendix III.
5.2LeadPaint Survey
LEI personnel inspected the Subject Sitefor previously unidentified lead painted materials.
None were observed; therefore no paint samples were collected.
