DBA HAMAKUA-KOHALA HEALTH has <br /> submitted an application for a Use Permit to establish a permanent medical clinic and caretakers' <br /> residence, including accessory structures and uses on 1.322 acres of land in the Single Family <br /> Residential-15,000 square feet(RS-15) zoning district and an application for a Special Permit to <br /> establish a transitional clinic and workforce housing, including accessory structures and uses on <br /> 4.321 acres of land in the State Land Use Agricultural District. The subject property is located at <br /> 54-3793 Akoni Pule Highway, at the northwestern corner of its intersection with Maluhia Road, <br /> Ohanaula and Honopueo,North Kohala, Hawaii, TMK: (3) 5-4-005:011. <br /> APPLICANT'S REQUEST <br /> 1. Request: The applicant is requesting the following entitlements to facilitate the development <br /> of the proposed Hamdkua Health Center: <br /> • PL-USE-2023-000010: Use Permit to allow the establishment of a permanent <br /> medical clinic and caretakers' residence, including accessory structures on an <br /> approximately 1.322-acre portion of a 5.682-acre property. <br /> • PL-SPP-2023-000038: Special Permit to establish a transitional medical clinic and <br /> workforce housing, including accessory structures and uses on about 4.321 acres of <br /> land in the State Land Use Agricultural District. <br /> 2. Proposed Development: The applicant has submitted two permit applications to establish <br /> the Hamdkua Health Center that will consist of a transitional medical clinic, permanent <br /> medical clinic and workforce housing in three (3)phases ("Project"). <br /> ■ Phase L• Phase I will consist of the construction of a transitional medical clinic, a <br /> single-story, approximately 4,153-square foot building located on the <br /> northwestern corner of the property. The existing Building C on the subject <br /> property will remain in its current condition and will be initially utilized for the <br /> live-in caretaker, who will provide evening security for the proposed Project and <br />
