Please review the enclosed Use Permit application and address each question separately and specifically. <br /> 2. Under item 3A, please provide a detailed written description of the proposed use, a statement of objectives and <br /> reasons for the request, including the square footage of the proposed structures. The times of each proposed <br /> use will be required,which includes but is not limited to the church services as well as the after-school children's <br /> programs,tutoring and supervised safe locations for the children of the nearby schools that is discussed further <br /> in the application. Also include a detailed description of the two phases and when each phase is expected to <br /> commence construction and be completed. Does the applicant currently operate their services from another <br /> location? If so, please include in your project objective. <br /> 3. Under item 3B, please provide a description of the subject property in sufficient detail to precisely locate the <br /> property. Describe existing uses, structures, and topography on the existing property. If portion of property is to <br /> be used, state the use of remainder of property. <br /> 4. Under item 3C,the following information should be added as they are land use designations that affect the <br /> application: <br /> a. The State Land Use Designation of the subject property is Urban. <br /> b. County Zoning is Single Family Residential-10,000 square feet(RS-10) <br /> c. The General Plan Land Use Pattern Allocation Guide (LUPAG) map designates the site as Low Density <br /> Urban. <br /> d. The South Kohala Community Development affects the property. <br /> e. The property is not within the Special Management Area (SMA). Please list how far the subject property <br /> is from the shoreline. <br /> 5. Under item 3D, please identify and discuss the surrounding uses of the neighboring parcels (i.e.,types of <br /> businesses, office, park use, residential uses, etc.) <br /> 6. Under item 3F, 3G, 3H, please provide information regarding any archaeological, historical and/or cultural <br /> resources on the subject property and floral and faunal resources, if any. <br /> a. Archaeological. Is there a previous archaeological study that was done for the site? Why was no <br /> archaeological study completed for the proposed project? <br /> b. Floral and Faunal. What was the site previously used as? What types of vegetation are located on the <br /> subject property? Which vegetation are considered endangered? In terms of faunal,what types of <br /> faunal have been located on the subject property? Of the faunal,which faunal are listed as endangered <br /> species? How often are these species located on the subject property? What types of impacts will the <br /> proposed project have on the floral and faunal resources? <br /> c. Cultural Resources.The Hawai'i State Supreme Court's "PASH" and "Ka Pa'akai 0 Ka 'Aina" decisions <br /> require decision-makers to consider a project's impact to native Hawaiian gathering and fishing rights. <br /> Specifically,there must be a discussion of the cultural, historical, and natural resources and associated <br /> traditional and customary practices of this site and the impact of this project to these resources and <br /> practices. <br /> d. Climate, Soil and Topography. Please provide a discussion on the Land Study Bureau Overall Master <br /> Productivity Rating for the subject property as well as the Agriculture Lands of Importance to the State <br /> of Hawaii classification of the subject property. <br /> 1 <br />
