CIS seeks to engage members in self-care and personal management by establishing a personalized housing support plan that is holistic and reflective of member's preferences and goals. CIS assists eligible Quest Integration (QI) members in becoming fully integrated members of the community as well as achieving improved health outcomes and life satisfaction. The goal of the Stabilization Intensive Case Management program (SICM) is to provide community-based beds for individuals experiencing emotional, behavioral, or psychological crises and who may be homeless, and/or medically vulnerable in a safe and therapeutic environment. The program provides adults, eighteen (18) years and older, a structured setting for a short-term stay for up to fourteen (14) days. The service components included in this program include Peer Support, Interim Case Management, and Care Coordination and Stabilization Intensive Case Management Program Funds. Representative Payee Staff assists consumers who suffer severe mental illness in managing Social Security Administration financial benefits by coordinating payments to monthly rent, utilities, food, clothing and medical needs. Remaining funds are then divided into “allowance” installments, which are distributed directly to the consumer on a periodic basis. Staff also negotiates with creditors on behalf of the consumer in order to coordinate payments to outstanding debt(s). Monthly budgets for each consumer are reviewed with the consumer by third-party, assigned case management staff, with the ultimate goal of transitioning consumers to financial self-management. RESOURCE CENTERS The Friendly Place Campus: Friendly Place Resource Center Provides a physical entry point for homeless and at-risk homeless persons and families needing assistance in the West Hawai‘i area. Service includes registering consumers with access to mail, emergency food, emergency supplies, phone, computer, fax, counseling and case management, meals, showers and laundry services. The program targets households experiencing homelessness. Housing Placement Program: Provides pre- and post- rental housing counseling, housing placement services to families eligible under the Temporary Assistance for Needy Families who possess a new Section 8 Housing Choice Voucher, or those transitioning from homelessness to permanent housing. Eligible families may be able to obtain assistance with initial move-in costs, such as rental deposit and rent, in order to help families, move directly into permanent housing. Permanent Supportive Housing: A program for persons who are chronically homeless. Applicants must consent to supportive services while receiving program assistance. Recipients generally pay 30% of household income towards rent, while the program subsidizes the contract rent remaining balance. Goals of the program are residential stability, increased income and/or skills, and greater self-determination. Support Services for Veteran Families HOPE has been selected by U.S. Veterans Initiative (U.S. VETS) to implement its Support Services for Veteran Families (SSVF) by assisting veteran households with emergency rent assistance, deposit, utilities or emergency needs that focus on either transition to a new permanent housing situation or retention of permanent housing. The goal of the SSVF program is to provide assistance to rapidly transition veteran families from homelessness and increase long-term housing retention. SUPPORTIVE HOUSING PROGRAMS These service-enriched affordable housing programs include housing, case management, community service, mail and computer access, as well as an array of on-site services such as employment and life skills training, mental health services, financial literacy training, counseling, and youth enrichment activities.
