5.In 1,000 characters,list the outputs to be completed.What are the quantifiable results of those activities that you would need to obtain in <br /> order to achieve the indicators you listed?These are OUTPUTS.Each output begins with a number and directly correlates to a specific activity. <br /> For example:delivering 100 STEM kits for students in grades K-8 by month 9. <br /> The goal of Sports as a Hook is to engage at-risk youth in a variety of beneficial academic and social activities,using athletics to grab their <br /> attention. <br /> This will be achieved through the following outputs: <br /> -850 students will have access to daily after-school enrichment by month 12 <br /> -d Parent and Family Engagement events will be held,one per quarter <br /> -18 ELA classes will be provided by month 12 <br /> -18 STEM classes will be provided by month 12 <br /> -30 students attend Camp Ready in June <br /> -250 students attend Summer Programs in June and/or July <br /> 6.In 1,000 characters,list the MEASUREABLE OUTCOMES to the public and your program participants based on your program's actions.How <br /> will you know when you are closer to your goal?What evidence would indicate this?These indicators are your measurable outcomes.Each <br /> statement should have a percentage.For example,rather than saying"we will conduct 10 community meetings"you could say"we will ensure <br /> 50%more of the target population will be better informed about XXX through pre/post surveys by month six.* <br /> The measurable outcome is that ASAS Hawaii participants will perform better than the general school population on academic indicators in <br /> ELA and Math.This will be assessed by comparing SBA performance and grades for ASAS Hawaii participants vs.the general school <br /> population. <br /> After-School All-Stars Hawaii-Academic Enrichment <br />
