Program Objectives & Performance <br /> The next set of questions will provide grant application reviewers a clear sense of what your proposal is designed to accomplish and what is <br /> needed to get there. The examples below relate to a fictional feeding program. Please keep in mind that character counts include punctuation <br /> and spaces. <br /> 1.In 500 characters,what is the community need the program intends to fulfill?Along with the narrative,please include data and/or citations <br /> documenting/measuring the need.* <br /> Decrease the incidence of fatal and non-fatal drowning in Hawaii Island youth.Drowning has been described as a"highly preventable"public <br /> health challenge.Drowning ranks among the top three causes of death from unintentional injury in most countries around the world. <br /> Drownings are the 5th leading cause of fatal injuries among Hawaii resident,with an average of nearly 40 deaths a year.A greater number of <br /> youth can participate in a water safety education program through a school environment. <br /> 2.In 500 characters,what is the program goal?Describe how your program advances.the CHANGE category you picked and creates a Ripple <br /> Effect of positive impact.What is the clear desired future state that correlates to the end of the one-year program period? <br /> The goal of the Alex&Duke De Rego Foundation's water safety education program is to promote drowning-prevention,public awareness and <br /> behavior-change for youth.Strengthen life skills by inspiring students to take an active role in the community while building leadership skills <br /> and encouraging teamwork.Thus inspiring youth and teachers to become better equipped in and out of the ocean.At the end of the program <br /> all students will have a better understanding of drowning prevention. <br /> • <br /> 3.In 1,000 characters,list what resources are needed to carry out your program,effort,or initiative.What do you need in order to carry out the <br /> activities at the frequency and scale needed to achieve the outputs?These are your INPUTS.Hint:this should look similar to the narrative <br /> section of the detailed budget request and include all types of in-kind services and contributions.* <br /> ADDF's water safety and ocean awareness education program's Interactive hands-on beach stations are conducted by certified lifeguards <br /> who provide their time and talent on their respected days off.Our education program requires all of our instructors to be certified lifeguards <br /> with training in first Aid&CPR.The budget reflects wages for 9 instructors for our nine week program.All students are required to wear <br /> uniforms(quick dry shirts)at all times during our beach activities.ADDF provides these uniforms to the student as well as all educational <br /> material which is reflected on the budget under supplies.With an anticipated nine schools in participation of our 2023/2024 water safety <br /> education program we intend to impact 700-800 youth annually.ADDF is currently working with DOE teachers creating our water safety <br /> education curriculum to be presented to the DOE for our ongoing program.Professional fees on the budget reflect this cost.Local construction <br /> companies provide bento's. <br /> 4.In 1,000 characters,list the MAJOR ACTIVITIES to be completed.To achieve the indicators you listed,what would you need to do?These <br /> action statements begin with a verb and normally includes a timeline.For example:design and collect materials for STEM kits by month 4.* <br /> ADDF's water safety education program is provided during the school year in the month of March thru May.Each school participates in a two- <br /> day session with Friday's being the day at the beach.Day 1-is done in the classroom and consist of a brief introduction of the ADDF staff, <br /> student watch documentary film,small group discussions of water related emergency situation to reduce injury,uniform and educational books <br /> distributed.Day 2-classes are transported to Hapuna Beach State Park on a field trip in tandem with their health and fitness education. <br /> Students get hands-on learning through rotating stations:exercise,swimming,water rescue,ocean awareness,First Aid/CPR.Certified <br /> Lifeguards as well as a teacher is present at each station.During our 9-week program with nine schools in attendance our organization is able <br /> to impact 700-800 youth.Every year students will take part in our water safety education that will cause lifesaving CHANGE for the betterment <br /> of our youth. <br /> 5.In 1,000 characters,list the outputs to be completed.What are the quantifiable results of those activities that you would need to obtain in <br /> order to achieve the indicators you listed?These are OUTPUTS.Each output begins with a number and directly correlates to a specific activity. <br /> For example:delivering 100 STEM kits for students in grades.K-8 by month 9.* <br /> ADDF's water safety&ocean awareness education program directly correlates with drowning and injury prevention.Drowning is described as <br /> "highly preventable"our program is designed to deliver annual water safety education to 6th grade middle schools.We intend to provide this <br /> education to the following schools:Waimea Public Charter School,Parker School,Kanu o ka A!na Charter School.Laupahoehoe School, <br /> Waikoloa Middle School,Kohala Middle School.Honokaa Middle School.Our education program will deliver water safety education annually <br /> with the ability to impact 700-800 youth every year.Our program is designed to be part of the middle school curriculum year after year creating • <br /> a culture of water safety education as a way of life! <br />
