REQUEST: <br /> Requesting a Resolution for the acceptance of a donation of 2 bottles of BioAstin per uniformed <br /> Firefighter employed by the Hawaii Fire Department from Cyanotech <br /> B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): <br /> Due to the physical nature of the job of a Firefighter, and the importance of staying physically fit and <br /> healthy, in an effort to help to support that need, Cyanotech (makers of BioAstin) has graciously <br /> offered to donate 2 bottles of BioAstin to each career uniformed Firefighter. <br /> They agreed to donate two (2) bottles of 60 count BioAstin 4MG vegan formula bottles per working <br /> uniformed firefighter, or a total of approximately seven hundred and ninety (790) bottles. The <br /> estimated retail value is $18,208.08. The estimated retail value may increase or decrease over <br /> time. <br /> This is a one time offer, however the MOU with Cyanotech is good for 1 calendar year, and within or <br /> by the end of that time frame, Cyanotech will determine if they would like to add to the offer or end it. <br /> SIGNED: DATE: Nov. 15, 2023 <br /> Department Head <br />
