Describe coordination with the Continuum of Care and efforts to address the needs of <br />homeless persons (particularly chronically homeless individuals and families, families with <br />children, veterans, and unaccompanied youth) and persons at risk of homelessness. <br />The OHCD continues to work and consistently coordinate efforts with the local Continuum of Care (CoC). <br />Bridging the Gap (BTG) is the Continuum of Care for the counties of Hawaii, Kauai and Maui. More <br />specifically, Community Alliance Partners (CAP) represents Hawaii County within BTG. <br />The CAP members consist of a wide variety of providers including advocates, community -based and <br />faith -based service providers and government agencies to address homelessness. In addition to the <br />organization -specific work each member achieves daily, the CAP coordinates the annual application for <br />federal and state funding and administers the annual`Point in Time Count 'The CAP meets on the last <br />Wednesday of each month both by video conference in Hilo and'Kond which'the OHCD attends regularly <br />to ensure collaborative and comprehensive efforts. \ \� <br />\ \ <br />The CAP shares four core beliefs: \ <br />1. Housing is a fundamental right; <br />2. Supports in housing, informed by the'unique strehgths and needs offeeaach household, help <br />sustain housing and realize the,best version of tliemselves � <br />3. People who have experienced homelessness are,experts'lin�heir own experience of <br />homelessness. <br />4. Respectful.engagement with others in the commum promotes CAP's shared belief that <br />homeressness is a community wide response. <br />Describe consultation with the Continuum(s)•of Care that serves the jurisdiction's area in <br />determining how to allocate ESG funds, develop performance standards for and evaluate <br />outcomes of'p ojects and activities assisted by ESG funds, and develop funding, policies and <br />procedures for thoperation and administration of HMIS <br />The OHCD is in regular contact with the CAP for ongoing efforts to overcome homelessness. When ESG <br />was awarded, the OHCD presented details such as funding amount, eligible activities, governing code of <br />federal regulations and deadlines regarding the ESG grant to the CAP.
