In 2,000 characters or less, describe the project. <br />Hawaii Island HIV/AIDS Foundation, doing business as Kumukahi Health + Wellness (KH+W), is seeking funds to <br />support Hawari Island Community Outreach through its PO'E Department (Prevention Outreach and Engagement). <br />This project will strengthen KH+W's risk reduction, public -benefit approach to HIV/STI (Human Immunodeficiency <br />Virus/Sexually Transmitted Infections) screening, prevention, counseling, training, and outreach for underserved <br />Hawaii Island communities. Through office -based and field outreach, KH+W will expand practical prevention services <br />that reduce harm, lower the risk of infectious blood -borne diseases such as HIV and viral hepatitis, and improve <br />connections to care. Funding will support the purchase and distribution of first aid kits and other basic health and <br />safety supplies during outreach, helping meet immediate needs while reducing barriers to engagement. It will also <br />expand prevention and screening capacity by purchasing HIV tests for outreach and prevention activities, with <br />participants connected to education, referrals, and follow-up services. In addition, funds will help build staff capacity <br />by supporting KH+W's HIV and Hepatitis C (HepQ Navigator's phlebotomy certification training, increasing the <br />organization's ability to provide responsive, field -based prevention services for confirmatory testing. Overall, this <br />funding will strengthen outreach -based prevention services, improve access to practical health resources, and support <br />a more effective, responsive Harm Reduction team serving Hawari Island residents with dignity, compassion, and <br />cultural humility. <br />* 43. 5O1(c)(3) Mission Statement <br />KH+W connects Hawaii Island's at -risk community with health equity and wellness through collaboration, advocacy, <br />and education, empowering people to live longer, happier, and healthier lives. <br />* 44. The proposed Kumukahi Community Outreach <br />Project will be carried out by the Prevention Outreach and Engagement Department, which consists of six staff based <br />in its Hilo and Kona offices, as part of the organization's risk reduction and prevention mission. KH+W provides a <br />range of services, including Med-QUEST navigation, overdose prevention, sexual health services, medical case <br />management, and risk -reduction outreach. Kumukahi's stated purpose is to serve underserved communities on Hawari <br />
