Please upload a completed CRF Grant Budget Form. <br />CRF Budget Form.xlsx <br />Public Purpose of Requested Funds <br />Per §2-138 of the Hawaii County Code 1983 (2016 Edition, as amended), the County may grant an award only if the <br />proposed program or service provides direct public benefits and fulfills a public purpose within the County. <br />46. According to §2-136, "public purpose" is defined as a benefit to, improvement of, or promotion of the <br />interests in the following areas. In 2,000 characters or less, describe the measurable outcomes you aim to achieve with these <br />funds. <br />Kumukahi Health + Wellness will provide a risk reduction, public -benefit approach centered on HIV/STI screening, <br />prevention, counseling, training, and outreach for underserved Hawaii Island communities. With this funding, the <br />organization aims to achieve the following outcomes: increase the distribution of basic health and safety supplies by <br />purchasing and distributing first aid kits during outreach. Success will be measured by the number of kits purchased, <br />the number distributed, and the number of outreach participants receiving supplies. The organization will expand <br />prevention and screening capacity by purchasing HIV tests. Success will be measured by the number of tests <br />purchased, the number used in outreach or prevention activities, and the number of participants connected to <br />education, referrals, or follow-up services as a result. KH+W will also build staff capacity in prevention services by <br />supporting its HIV and HepC Navigator's phlebotomy certification training. Success will be measured by enrollment in <br />the certification course, completion of training, and increased ability to support field -based prevention services. <br />Additionally, KH+W will strengthen connections to care for underserved community members reached through <br />outreach. Success will be measured by the number of individuals engaged, the number referred to prevention or care <br />services, and the number successfully linked to follow-up support. Overall, these funds will help improve access to <br />practical health resources, strengthen outreach -based prevention services, and increase the effectiveness of the Harm <br />Reduction team, resulting in a more responsive community outreach program that better meets immediate health <br />needs and improves pathways to care for Hawaii Island residents. <br />
