On March 9,2021, approximately 400,000 gallons of sewage was discharged via a <br /> drainage swale to the coastal waters fronting Papa'ikou Wastewater Treatment plant, <br /> caused.by heavy rain. <br /> c. On November 30,2020,approximately 28,000 gallons of sewage spilled from the force <br /> main along Banyan Drive in Hilo near the Banyan pump station,reaching the Pacific <br /> Ocean.The spill occurred when a contractor working on the sidewalk accidently • <br /> punched a hole into the force main. <br /> d. On July 10-11,2020,approximately 20,000 gallons of sewage spilled from the force <br /> main at the Keopu pump station along Ali'i Drive in Kailua-Kona,reaching the Pacific <br /> Ocean.The spill was due to a failure in the cast iron force main caused by corrosion. <br /> e. On September 28,2017,approximately 5,000 gallons of sewage spilled from the <br /> gravity sewer line in Naalehu,discharging to the ground without reaching surface <br /> waters.The spill was due to a badly corroded sewer line. <br /> f. On June.13,2017,approximately 2,000 gallons of sewage spilled from the force main <br /> along Ali'i Drive in Kailua-Kona near the Lanihau sewage pump station,reaching the <br /> Pacific Ocean. The spill was due to a rupture in the cast iron force main caused by <br /> corrosion. • <br /> g. On June 5,2017,a pipe failure caused secondary treated and disinfected wastewater <br /> • to discharge to coastal waters near the Pua sewage pump station. <br /> h. On April 3,2017,approximately 41,000 gallons of sewage spilled from the manhole at <br /> Waianuenue and Laimana Street intersection in Hilo, reaching Wailuku River and the <br /> Pacific Ocean. The spill was due to debris blockage in the gravity sewer line. <br /> i. On January 27,2017,approximately 5,000 gallons of sewage spilled from the Paukaa <br /> pump station along Paukaa Drive in Hilo,reaching the Pacific Ocean.The spill was due <br /> to rainfall overwhelming the capacity of the pump station which led to the wet well <br /> overflowing. <br /> 25. Based on review of the records provided by Respondent and observations during <br /> inspections,Respondent does not have a program in place to systematically repair,rehabilitate, <br /> and replace aging infrastructure prior to failure. <br /> 26.
