AGING <br />AGING <br />Non -Contracted Services (continued) <br />d. Provide clients served the opportunity to evaluate the program through a survey of <br />which at least 70% of safety deficits identified in the service provider's <br />assessment were corrected with program services and in-home safety items. <br />90% of clients indicated safety deficits were corrected with program services <br />and in-home safety items. (In some situations, the corrective item would be <br />too costly and not feasible for this program i.e. bathroom renovation). <br />Services Contracted to Private/State Agencies <br />Case Management. <br />Provide clients served the opportunity to evaluate the program through a survey of <br />which at least 70% of clients surveyed will indicate receiving home and community <br />based services improved their ability to remain in their homes. <br />Information not available at this time. A client survey will be conducted in April <br />for the Case Management program. <br />2. Kupuna Care. <br />Provide clients receiving Kupuna Care Services (adult day care, personal care, <br />home -delivered meals, attendant care, chore, home/housekeeper, and assisted <br />transportation services) the opportunity to evaluate the program through a survey of <br />which at least 75% of clients surveyed will show an improved ability to remain in <br />their own homes. <br />Information not available at this time. A client survey will be conducted in April <br />for the Kupuna Care program. <br />Legal Services. <br />a.
