<br /> If awarded a grant from the Department of Finance, the grantee will be expected to sign <br /> <br /> an agreement/letter of understanding abiding by certain terms and conditions, including: <br /> A final report detailing expenditure of funds and project results is required within 60 <br /> <br /> days of project completion. We reserve the right to request documentation such as <br /> receipts. <br /> ' In all printed matter, press releases, and other local announcement regarding this <br /> project, the grantees shall acknowledge the funding support provided by the Spay-Neuter <br /> Special Fund. <br /> G Monies not used for the purpose intended shall be returned to the Spay-Neuter Special <br /> Fund administered and managed by the Department of Finance. <br /> ~l Statistical Follow Up Questionnaire will be sent and required to be completed and <br /> returned within 60 days. <br /> PROPOSAL EVALUATION PROCESS <br /> The Spa}' Neuter Specia] Fund Committee will screen and evaluate proposals and notify <br /> applicants by April 15th. Any proposal that does not have the required information or <br /> documentation may be disqualified. The criteria that will be used to evaluate proposals <br /> include: <br /> Goal/Scope: The proposal demonstrates how it will benefit specific animals and reach its <br /> audience, both animals and people. Proposals that focus on feral cats should specify how <br /> attempts will be made to impact the entire colony/population/community. <br /> Need: The proposal conveys a good understanding of a particular need in the targeted <br /> area and adequately addressed how the proposed project addresses it. <br /> Project design: The project iswell-thought out and presents a clear method/strategy to <br /> reduce pet overpopulation. Ability to define and quantify pet overpopulation problem in <br /> your community. <br /> Collaboration: The proposal takes into consideration any possible collaboration (with <br /> animal shelters, veterinarians, educational outreach providers) that can effectively help <br /> reach the proposal's goal. <br /> Assessment: What measurable criteria will be used to determine if your organization <br /> reached its objectives. <br /> ENCLOSURES <br /> Please include the following: <br /> 1. Copy of your 501(c)(3) determination letter <br /> 2. Documentation of registration with the State of Hawaii as a nonprofit <br /> organization <br /> 3. Annual Report, brochure or other promotional material relevant to evaluation of your <br /> proposal <br /> 4. Financial statements for the last fiscal year (form 990 or 990EZ) if required to be filed. <br /> 5. Written statement required for omitted documents listed above <br /> 2 <br /> <br />
