AGING <br />AGING <br />Non -Contracted Services <br />1. Caregiver Support: National Family Program <br />a. Develop and maintain information and outreach displays and presentations that <br />provide the public and caregivers with information on resources and services <br />available within the community (i.e. health fairs, community services fairs, <br />caregiver conference and workshops or meetings.) Participate in at least 10 <br />activities throughout the County per fiscal year and, when feasible to collect <br />evaluation, at least 70% will respond favorably. <br />Five activities/presentations completed (two conferences and three outreach <br />presentations). <br />b. Establish trained individuals in various districts of the island to assess caregiver <br />needs and provide information and assistance on the services available within <br />their community and link the caregiver to the services and opportunities that are <br />available. At least 70% of caregivers will report that services and programs <br />funded by the National Family Caregiver Support Program (NFCSP) helped them <br />continue to give care. <br />Assessments and information and assistance are executed currently by <br />existing agencies/providers, island wide, and caregivers have immediate and <br />convenient response to their needs. With the current process, 90% of <br />caregivers had assessments completed within the time requested by the <br />caregiver. <br />2. Caregiver Support, Education, and Training: Caregiver Resource Center. <br />a. Provide caregivers with access to resource materials available in the resource <br />center. At least 30% of caregivers served will report that the resource materials <br />improved their knowledge of caregiving. <br />83% of clients and caregivers that used the resource materials (books, <br />brochures, videos, etc.) acknowledged the increased knowledge and <br />understanding of their situation or are better informed about the condition <br />of the person in their care. <br />Six Month Progress Report on Program Objectives for FY 2005-06 <br />
