STANLEY <br />DEPUTY DIRECTOR <br />01 r?, <br />,k <br />u <br />SUBJECT: FY 1999 - SUPPLEMENTAL AGREEMENT NUMBER 2 RELATING TO <br />NEGOTIATED AGREEMENT FOR SERVICES (GOCY 96-08-05) <br />Enclosed is the Negotiated Agreement for Services between the Office of <br />the Mayor and the Department of Human Services. The Supplemental Agreement <br />Number 2 to provide an Early Childhood and Care Coordinator for the children and <br />families of the County of Hawaii, reflects a change in the Scope of Services, and <br />extends the time schedule from July 1, 1998 through June 30, 1999, with the <br />compensation amount of $57,000. <br />Please review the Agreement, sign the signature pages, notarize the - <br />Contractor's Acknowledgement page of the Agreement (tabbed pages), and <br />return them to our office.
