REQUEST: <br /> <br /> 1) Requesting a resolution authorizing the return of $31,834.34 of unspent funds from the Hawai'i County <br /> Nutrition Program to the Hawai'i County Office of Aging due to early termination of the Healthy Aging Project. <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): <br /> <br /> The County of Hawaii was awarded grant funds from the Administration on Aging to coordinate the <br /> mobilization of the public and private partnerships at the community level to implement and sustain the <br /> delivery of evidence-based prevention through an aging service provider organization. <br /> <br /> The Hawai'i County Office of Aging (HCOA) contracted with the Hawai'i County Nutrition Program (HCNP) to <br /> implement EnhanceFitness, an evidence-based exercise program, in two nutrition sites. For a number of <br /> reasons EnhanceFitness did not prove to be a very good fit for the nutrition sites, and a difficult decision was <br /> made to terminate the program. <br /> The HCOA is committed to maintaining or improving the health status of Hawai'i County's elderly citizens. To <br /> this end, HCOA requested approval from the State Executive Office on Aging (EOA) to explore the possibility <br /> of implementing the Stanford University Chronic Disease Self Management Program, another grant <br /> approved evidence-based prevention.
