Attachment 1 <br />2. HCPD shall ensure all objectives outlined in the SDAT proposal are completed <br />(See Attachment 6) <br />3. HCDP shall work in collaboration with DOH HHI to: <br />a. Identify ongoing opportunities for collaboration in the promotion of <br />community designs that encourage physical activity and healthy eating <br />b. Provide input that would be incorporated into the EDH Action Plan <br />Update <br />4. HCDP shall provide quarterly progress reports utilizing an HHI Evaluation Team <br />developed template <br />5. HCDP shall provide a final report to DOH HHI upon completion of the <br />Agreement which shall at minimum include a copy of the final EDH Action Plan <br />Update. <br />DOH REF. NO. HHI-09-04 <br />
