Summary of Service <br /> The Long Term CAre Access Project(LTCAP), a short term case management <br /> service, provides regular contact with the frail, vulnerable elderly as identified by the <br /> Hawaii County Office of Aging. The project conducts updates of registration forms, <br /> provides in-home nursing assessments, information and referral services, and limited <br /> interventions. The program also orders, verifies, and funds personal care services <br /> for eligible clients. <br /> II.
