Community Planning and Development - Partnerships <br /> Healthy Aging Initiative <br /> Establish a Healthy Aging Partnership with public and private agencies and elderly <br /> consumers. <br /> Support and coordinate a chronic disease self management program where at least <br /> 75% of participants surveyed will indicate that the self management program <br /> improved or maintained their health status. <br /> <br /> As of September 1, 2008, a Hawaii County Healthy Aging Partnership was <br /> established to implement and embed an evidence-based self management <br /> program for older individuals. With Hawaii County Office of Aging providing <br /> leadership, partners include Alu Like, Inc., Bay Clinic, Inc., West Hawaii <br /> Community Health Center, Services for Seniors, Kau Rural Health Community <br /> Center, and YMCA - Waiakea Settlement. <br /> The evidence-based program is Stanford's Chronic Disease Self Management <br /> Program (CDSMP). It is a 2-1/2 hour once a week for 6 weeks program with a 6 <br /> month followup. As of November 30, 2008, 35 older individuals have completed <br /> the program and are scheduled to be re-evaluated in April and May 2009. <br /> <br /> 4. Database Development and Maintenance <br /> a. Collaborate with private and public sectors to provide training and technical <br /> assistance to HCOA service providers on data collection and reporting. At least <br /> one training meeting will be conducted during the first half of the program year. <br /> <br /> Due to the planned move into the new facility at 1055 Kinoole Street, HCOA <br /> data staff met with Service Provider staff individually to provide technical <br /> assistance, and training to new staff. This approach was taken with the plan <br /> of holding a larger meeting with all service providers, once HCOA settles <br /> into its new facility where training room facilities will be available and <br /> logistically easier to arrange. <br /> <br /> b. Respond to and complete all data/information requests in a timely manner. 90% <br /> of data/information requests will be completed within 10 working days of receipt <br /> of request. <br /> <br /> HCOA data staff responded to four data request during the first half of the <br /> year. Three requests were from the State Executive Office on Aging, <br /> requesting demographic data on consumers served in specific services in the <br /> previous Fiscal Year. One request by the editor of the Silver Bulletin on the <br /> number of seniors who were born in the Chinese Year of the Ox for a feature <br /> story. <br /> <br /> <br /> Six Month Progress Report on Program Objectives for FY 2008-09 3 <br />
