East Hawaii Mobile Care Health - Van Purchase (CDBG): <br />Acquisition of a mobile care vehicle which will provide <br />dental, medical, health and social services for a <br />minimum of 1000 homeless and low income persons <br />annually in East Hawaii. <br />2. Hamakua Health Center - Vehicle Purchase (CDBG): This <br />program will benefit 8540 residents of North Hilo to <br />Hamakua of which 5570 or 65.22% are low and moderate <br />income by providing access to health care. <br />3. Hamakua Assisted Living Facility - Feasibility Study <br />(HOME): To conduct a study to determine if it is <br />feasible to develop an assisted living facility in <br />Hamakua. The facility will address the needs of low - <br />and very -low income elderly residents who desire to <br />live independently within their communities,.yet <br />require assistance in performing certain daily <br />activities. <br />4. <br />Provide funds for operational expenses (equipment, <br />telephone, supplies, insurance, mileage,'fuel, and <br />utilities) to provide shelter and support services to <br />approximately 270 homeless persons annually. <br />5. West Hawaii Family Crisis Shelter (ESG): to purchase a <br />12 -passenger van and pay for insurance to provide pick <br />up and transportation services to approximately 261 <br />homeless adults and children who are battered or <br />fleeing physical and/or sexual abuse. <br />6. Hilo Ohana Snouse Abuse Shelter - Van Purchase (ESG): <br />Provide funds for operational expenses (repair and <br />maintenance of vehicle and insurance) to provide <br />shelter and support services to approximately 360 <br />homeless adults and children who are.battered or <br />fleeing physical and/or sexual abuse. <br />V - 11 98 Consol/3-9-98 <br />
