Transportation. <br /> Provide transportation service to congregate meals participants in order for them to <br /> receive nutritious meals and participate in site activities that enhances physical and <br /> social well- being. <br /> a. The percentage of participants identified as non - nutritional risk at the <br /> beginning of the program year shall be maintained at the same or higher levels <br /> by the end of the program year. <br /> b. The percentage of participants identified as high nutritional risk at the <br /> beginning of the program year shall be maintained at the same or lower levels <br /> by the end of the program year. <br /> During the first six months of Program Year 2009 -10, 376 unduplicated older <br /> individuals were provided with transportation services to participate at a <br /> congregate Approximately re ate meal site. A roximatel 8 out of ten (81 %) of the total participants <br /> completed a nutritional risk survey. During the same reporting period, the average <br /> monthly proportion of participants who were identified as nutritionally at high risk <br /> was 24% while 63% was the average monthly proportion of participants who were <br /> identified as non - nutritionally at -risk. 28% were identified as nutritionally at risk <br /> in July and dropped to 22% in December 2009.
