Provide access to all districts of the island, trained individuals to assess caregiver <br /> needs, provide information and assistance that link caregivers to needed services <br /> and provide follow -up on an as- needed basis. At least 80% of requests for <br /> information and/or services will have immediate (within 7 days) contact with an <br /> assessor to begin the assessment process and identify caregiver needs. <br /> li During the first six months of FY10, 87 clients received 485 units of Access <br /> services through the National Family Caregiver Support Program. 95% of <br /> requests for information and / or services received an immediate contact to <br /> begin the assessment process in order to identify caregiver needs. <br /> 3. Community Planning and Development — Partnerships <br /> Healthy Aging Initiative <br /> a. Increase by 40% the number of chronic disease self management workshops <br /> offered in Hawai`i County. The 40% increase would be based on the number of <br /> workshops completed for the fiscal year ending June 2009. The intent is to <br /> embed a health related evidence -based program in Hawai`i County. <br /> Six workshops were completed in FY 2009. For the period July to December <br /> 2009, one leadership workshop was completed with 14 graduates and one <br /> Chronic Disease Self Management Workshop was started in December and <br /> ended in January. Other new Lay Leaders scheduled workshops to begin <br /> January 2010. <br /> b. Support and coordinate a chronic disease self management program where at least <br /> 75% of participants surveyed will indicate that the self management program <br /> improved or maintained their health status. <br /> The required 6 month followup surveys for completed workshops will be <br /> conducted in the next 6 month period. <br /> Six Month Progress Report on Program Objectives for FY 2009 -10 3 <br />
