COUNTY OF HAWAII <br />RESOLUTION NO. 344 10 <br />STATE OF HAWAII <br />RESOLUTION AUTHORIZING THE OFFICE OF THE MAYOR TO ENTER <br />INTO AN AGREEMENT WITH THE STATE OF HAWAI'I DEPARTMENT <br />OF HEALTH, PURSUANT TO HRS 46 -7, FOR THE HOSPITAL <br />DISCHARGE PLANNING MODEL AND THE AGING AND DISABILITY <br />RESOURCE CENTER EXPANSION GRANTS <br />WHEREAS, the Hawaii County Office of Aging recently partnered with the <br />Department of Health, Executive Office on Aging in applying for federal grants; and <br />WHEREAS, the County was awarded federal funds for the Hospital Discharge <br />Planning Model grant and for the Aging and Disability Resource Center (ADRC) <br />Expansion grant; and <br />WHEREAS, the Hospital Discharge Planning Model grant is to develop a model <br />that 1) puts the patient and caregiver(s) at the center of the discharge planning process, <br />2) focuses on discharging patients home with community -based services, via the <br />ADRC, 3) reduces the number of patients retained in acute care beds past the point of <br />clinical discharge, and 4) reduces the number of default discharges from acute care <br />units to nursing facilities; and <br />WHEREAS, the ADRC Expansion grant objectives are to develop a five (5) year <br />operational plan and budget for achieving statewide coverage of Hawai'i's ADRC, and to <br />expand and formalize linkages with key aging, disability, and health care providers; and <br />WHEREAS, in addition to receiving these federal funds, the State Department of <br />Health, Executive Office on Aging is also providing state general funds for ADRC <br />operations; and <br />WHEREAS, Hawai'i Revised Statutes, Section 46 -7, requires that county <br />departments obtain the consent of the council to enter into agreements with the federal <br />or state governments respecting action to be taken pursuant to any of the powers <br />granted by law to furnish, expend, and receive any funds or other assistance in <br />connection with projects being or to be undertaken pursuant to those powers. <br />
